Dear Sir or Madam:

This spill report suicmarizes the oil spill release incident that occurred at the Fancy Point Switch Yard, Entergy Operations, Inc., River Bend Station site, St. Francisville, Louisiana, West Feliciana Parish, on September 19,1997. This complies with written report requirements of Section 10111.D. of the Louisiana State Police - Hazardous Material Information Development, Preparedness, and Response regulations.

At approximately 10:16 p.m. on the night of September 19, a 400 MVA transformer number 9412 at Fancy Point Switch Yard tripped due to intemal failure. The internal failure subsequently resulted in the rupture and loss of approximately 5000 gallons of mineral oil spilling over the graveled yard areas, and into concrete lined cable trays.

Entergy personnel promptly began oil spill containment and control activities, together with immediately mobilizing a local hazardous material emergency response contractor, LATICO Environmental Ser*es. LATICO arrived promptly at the location to furthe:

assist with oil spill containment and initiated spill cleanup and remediation activities.

Clean-up of the spill site is expected to be completed on September 23,1997.

Environmental Safety Section at 2:00 a.m. to JetT Cagle (incident no. 97-4163); 4) the West Feliciana Parish Emergency Planning Committee / West Feliciana Sheriffs Office at 2:05 a.m. to Earl Reech; 5) the U.S. Environmental Protection Agency, Region VI at 2:15 a.m. to Althea Fostert and 6) the Waterworks Warning Network-Lower Mississippi River at 3:22 a.m. to Mert Fagan. q s

The attached laboratory analysis results of the oil indicates it is non-PCB Soil and debris from the cleanup, including the oil absorbing materials, will be properly disposed of at the Waste Management Woodside Landfill in Walker, Louisiana. Approximately 2000 gallons of waste oil was reclaimed and disposed of through a local recycler. There were no fire or injuries associated with this incident. No oil entered the storm drains on the switch yard, or posed any threat to waters of the state.

Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate and complete.
