Dear Ms. Levy:

This letter is being written in response to the telephone conversation I had with Kentya Hart on 6/11/98 concerning the missed permit sample for outfall 002, Permit No.'s NPDES LA0042731 and LWDPS WP0409. Due to infrequent discharges from permitted outfall 002 during the month of May, a sample was inadvertently missed during a discharge of 19,575 gallons of reject water from a water demineralizer unit used at the station. Station personnel failed to adequately communicate that a ,

discharge of ionic reject water was occurring on May 19,1998. As a result, during -

preparation of DMR's for May 1998 it was discovered that a discharge had occurred I without taking a sample. This reject water (outfall 002) normally flows to the Mississippi River via outfall 001. Recent ionic reject samples during the months of March and April 1998 showed very low TSS and O&G analytical results which is typical for this outfall. Based on historical data, there were no probable permit limit-f exceedances during this discharge. To prevent this incident from happening again, ]; (/ 7 3 procedural notification requirements will be reemphasized during upcoming training and administrative processes will be adjusted to compensate for any inadequacies concerning notification procedures during discharges.
