Dear Mr. Givens:

Water Discharge Permit No. WPO409 NPDES Permit No. LA0042731 Analysis of a sample collected on October 24, 1986 at Outfall 005 of the above referenced permit indicated a Total Suspended Solids (TSS) of 150 mg/l which exceeded the permitted limit of 135 mg/1.

The high TSS was caused by intrusion of silt-laden stormwater runoff into the aeration basin. Accumulations of silt had-reduced the capacity of ditches which normally divert stormwater around the basin.

The discharge from the basin was stopped until the stormwater diversion was re-established to prevent a recurrence.

I certify under ' penalty of law that this document and all attachments were prepared under the direction or supervision in accordance with a system designed to assure that qualified personnel properly gather and evaluate the information submitted.

Based on my inquiry of the person or persons who manage the system, or those persons directly responsible for gathering the information, the information submitted is, to the best of my knowledge and belief, true, accurate, and complete. I am aware that there are significant penalties for submitting false information including the possibility of fine and imprisonment for knowing violations.
