7-SPECIAL REPORT SR-86-003-00 Faulty Sample Line Check Valve Rendered Wide Range Gas Monitors Inoperable Greater than Seven Days INTRODUCTION At approximately 0849 hours0.00983 days <br />0.236 hours <br />0.0014 weeks <br />3.230445e-4 months <br /> on May 23, 1986 Waterford Steam Electric Station Unit 3 was operating at 100% reactor power when operations personnel declared Wide Range Gas Monitor (WRCM), PRM-IR-0110, inoperable. In accordance with Technical Specification 3.3.3.1 Action 27, Health Physics personnel established the preplanned alternate method for monitoring plant stack emissions as defined in procedure EP-2-050, "Off-Site Dose Assessment (Manual)".

Since the monitor could not be returned to service within seven days, this report is being submitted pursuant to Technical Specification 3.3.3.1 Action 27 and 6.9.2.

NARRATIVE on May 23, 1986 Waterford Steam Electric Station Unit 3 was operating at 100%

reactor power when Instrument and Controls (I&C) personnel were performing a functional test, as described in procedure MI-3-382, " Plant Vent Stack High Range Noble Gas Radiation Monitor Functional Test PRM-IR-0110", on the middle to high range portion of the Wide Range Gas Monitor-(WRGM). As part of the surveillance, I&C personnel are to ensure that the flow through the sample line can be controlled at approximately 0.072 standard cubic feet per minute (scfm).

However, the highest observed flow was only 0.06 scfm. Therefore, at 0849 hours0.00983 days <br />0.236 hours <br />0.0014 weeks <br />3.230445e-4 months <br /> on May 23, 1986 operations personnel declared the WRCM inoperable. In accordance with Technical Specification 3.3.3.1 Action 27, Health Physics personnel established the preplanned alternate method of monitoring plant stack emissions as defined in procedure EP-2-050, "Off-Site Dose Assessment (Manual)".

s The low flow rate was attributed to a faulty check valve. Since the check valve was sticking, desired flow rate could not be established in the sample line. Maintenance personnel removed the check valve, reworked the seat, and on June 4, 1986 satisfactorily tested the WRGM in accordance with the previously described procedure.- At 1958 hours0.0227 days <br />0.544 hours <br />0.00324 weeks <br />7.45019e-4 months <br /> on June 4, 1986 operations personnel returned the WRGM to service. Since the monitor was not returned to service before the end of the seven day limit (May 30, 1986), as defined in Technical Specification 3.3.3.1 Action 27, this Special Report is submitted pursuant to Specification 6.9.2.

Since Health Physics personnel had an alternate method of monitoring plant stack emissions, and since the low range detector was operable, the inoperability of the Vide range monitor did not pose a safety concern.
