' Faulty Low Range Detector and Circuit Board Rendered A Wide Range Gas Monitor Inoperable Greater than Seven Days INTRODUCTION At approximately 1925 hours0.0223 days <br />0.535 hours <br />0.00318 weeks <br />7.324625e-4 months <br /> on May 27, 1987 Waterford Steam Electric Station Unit 3 was operating at 100% reactor power when operations personnel declared Condenser Vacuum Pump Discharge Wide Range Gas Monitor (WRCM), PRM-IRE-0002

. inoperable. In accordance with Technical. Specification 3.3.3.1 Action 27, Health Physics personnel established the preplanned alternate method.for monitoring Condenser Vacuum Pump discharge exhaust as defined in procedure EP-2-050, "Off-Site Dose Assessnent (Manual)".

Since the monitor could not be returned to service within seven days, this report is being submitted pursuant to Technical Specification 3.3.3.1 Action 27 and 6.9.2.

NARRATIVE On May 27, 1987 Waterford Steam Electric Station Unit 3 was operating at 100%

reactor power when Instrument and Controls (I&C) personnel were performing a functional test, as described in procedure MI-3-387, " Condenser Vacuum Pump Discharge Wide Range Noble Gas Radiation Monitor Channel Calibration PRM-IR-0002", on the Condenser Vacuum Pump Discharge Wide Range Gas Monitor I (WRCM). During the surveillance, the Low Range channel failed the checksource test. Therefore, at 1925 hours0.0223 days <br />0.535 hours <br />0.00318 weeks <br />7.324625e-4 months <br /> on May 27, 1987, operations personnel declared the WRGM inoperable. In accordance with Technical Specification 3.3.3.1 Action 27 Health Physics personnel established the preplanned alternate method of monitoring Condeaaer Vacuum Pump discharge exhaust as defined in procedure EP-2-050, "Off-Site Dose Assessment (Manual)".

4 The failed checksource test was attributed to a failed input / output circuit

. board and low range detector. After replacement, the monitor failed to perform properly and troubleshooting was resumed. The replacement input / output circuit board was discovered to be faulty and due to the difficulty in finding this problem, the monitor was not returned to service within the seven day limit.

Repairs were completed on June 6, 1987, and the WRGM was returned to service in accordance with the previously described procedure. Since the monitor was not returned to service before the end of the seven day limit (June 3, 1987), as defined in Technical Specification 3.3.3.1 Action 27, this Special Report is submitted pursuant to Specification 6.9.2.

Since Henith Physics personnel had an alternate method of monitoring Condenser Vacuum Pump discharge exhaust the inoperability of the wide range gas monitor did not pose a safety concern.
