APPENDIX B l Cooper Energy Services Docket No. 99900317/87-01 NOTICE OF NONCONFORMANCE l

On the basis of the results of an NRC inspection conducted on March 30-April 3, 1987, it appears that certain of your activities were not conducted in accordance with NRC requirements. S l

Criterion V of Appendix B to 10 CFR 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accompli s hed.

(1) Cooper Energy Services Quality Control Procedure QCP-10-6, Section 4.1.2, Item 1, requires a work order (form #451) to be generated for rework or repair of nonconforming materials, parts, or components.

Cooper Energy Services Quality Control Procedure QCP-10-13 Section 3.2, requires, in part, that documents of significant value maintaining, reworking, repairing, replacing, or modifying an item or that would be of Significant value in determining the cause of an accident or malfunction of an item shall be maintained until the item to which they pertain are decommissioned from service.

Contrary to these requirements, no work order was available for Material Review Requests (MRRs) 17053, 10328, 2912, and 12765. (87-01-02)

(2) Cooper Energy Services Quality Control Procedure QCP-10-15, Section 4.11, requires that the calibration frequency be provided in Inspection Procedure ISP-5-2.

Contrary to this requirement, no calibration frequency was provided for hardness testing equipment. (87-01-03)

(3) Cooper Energy Services Quality Control Procedure QCP-10-6. Section 4.5.2, requires that documents referenced by the Inspection Plan be on file. In addition, Cooper Energy Services Material Specification SE-89-2N, Section 3.1, requires that vendors performing plating in accordance with SE-89-2N provide a certificate of conformance.

Contrary to these requirements, no certificate of conformance was available for plating repairs that were called for in MRR 17053. (87-01-04) 8707140012 070708 PDR GA999 EMVCOIN PDR 99900317
