APPENDIX B Allied Nut & Bolt Company, Incorporated Docket No. 99901093/87-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted June 23-26, 1987, it appears that certain of your activities were not conducted in accordance with hRC requirements.

A. Criterion V of Appendix B to 10 CFR Part 50 states, in part, " Activities .

affecting quality shall be prescribed by documented instruction, proce- I dures...and shall be accomplished in accordance with these instructions, procedures,...."

Paragraph 3.2 of the ANB Quality Assurance llanual, QAP 2.0, Revision 1, states, in part, "The personnel assigned are qualified in accordance with the guidelines established by ANSI N45.2.6 (1978). They are also required to be trained in the requirements of this manual consistent with the work they are performing...." The personnel certification l form, states, in part, "This Is To Certify That...Has Been Evaluated and is considered Qualified to perform the Specific Assigned Tasks...in ccmpliance with ANSI N45.2.6 (1978)."

i Contrary to the above, at the time of the inspection, there was no objective evidence that ANB had a systematic method and/or procedures for training and c,ualifying personnel responsible for quality control and/or assurance activities. Specific guidelines in order to establish the qualifications necessary to certify personnel according to the AND certification system (Levels 1 II, and III) did not exist. (87-01-03)

D. Criterion VII of Appendix B to 10 CFR 50 states, in part, " Measures shall be established to assure that purchased material, equipment, and services, whether purchased directly or through contractors and subcontractors, conform to the procurement documents. These measures stall include provisions, as appropriate, for source evaluation and selection, objective evidence of quality furnished by the contractor or subcontractor, inspection at the contractor or subcontractor source, and examination of products upon delivery."

Paragraph 2.3 of the AhB QAP 7.0, Revision 2, states, "The supplier's quality assurance capabilities shall be approved before he is awarded a purchase order."

Contrary to the above, no source evaluation was performed on Pottstown Plating, which provides a special process service to ANB. (87-01-04) 01 $k

C. Criterion XV of Aprandix B to 10 CFR Part 50 states, in part, "Noncon-forming iteus shall be reviewed and accepted, rejected, repaired, or reworked in accordance with documented procedures."

Paragraph 3.4 of the ANB QAP 15, Revision 2 states, in part, "The General Manager will disposition the NCR and return the original to the QC Supervisor. The disposition will be either:

a. Accept as is...

b. Rework...

c. Scrap...

d. Return to supplier or manufacturer,,.."

Contrary to the above, numerous nonconformance reports were determined to have final closecut signatures with no disposition checked. (87-01-05)
