APPENDIX A Sandvik Special Metals Corporation Dccket No. 99900764/87-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on March 16-19, 1987, it appears that certain of your activities were not conducted in accordance with NRC requirements.
Criterion V of Appendix B to 10 CFR Part 50 states- " Activities affecting ouality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative. or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
- 1. Sections 3.5.1 and 3.5.2 of QA-GA-5, " Receiving Inspection and Control of Starting Material," Revision 14, dated February 7,1986, requires, in part, that material be inspected to determine conformance to attribute limits specified on forni QC053. Form QC 053 requires that the inside diameter (ID) dimensions of hollows be inspected with a go-no go gauge measuring 1.625 and 1.655 inches.
Contrary to the above, the ID of 19 hollows from ingot number 2241900 were being inspected with a go-no go gauge measuring 1.615 and l
1.665 inches. (87-01-01)
- 2. Section 2.1.1.8 of Z121, " Alkaline Cleaning," Revision 10, dated June 3,1985 requires that the date that Tank #3 was filled with fresh cleaning solution be written on a piece of masking tape and placed on the west end of the 16nk.
Contrary to the above, the west end of Tank #3 did not exhibit a piece of masking tape or any other ioentification concerning the filling of Tank f3 with fresh cleaning solution. (87-01-02) l l
