APPENDIX A NOTICE OF VIOLATION TU Electric Dockets: 50-445/89-35 Comanche Peak Steam Electric Station Permits: CPPR-126 Unit 1, Glen Rose, Texas During an NRC inspection conducted on May 3 through June 6, 1989, a violation of NRC requirements was identified. In accorc'ance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

Criterion III of Appendix B to 10CFR Part 50 and Section 3.0 of TU Electric QA Manual require that design changes, including field design changes, shall be subject to design control measures commensurate with those applied to the original design and be approved by the organization that performed the original design.

Contrary to the above, in 1987 TU Electric maintenance substituted carbon steel Grade 5 bolts for silicon bronze bolts in the auxiliary feedwater pump motor without documenting, reviewing, and approving the change to the design.

This is a Severity Level IV violation (Supplement II)
