EA 88-233 NOTICE OF VIOLATION During of NRCan inspection conducted requirements on June 27, 1988 through July 1,1988 violations were identified.

In accordance with the " General Statement of Policy and Procedures for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988) the violations are listed below:

Section 21.21, " Notification of Failure to Comply or Existence of a Defect," of 10 CFR Part 21 requires, in part, that each individual or other entity subject to the regulations adopt appropriate procedures to provide for evaluating deviations or inferming the licensee or purchaser of tne ceviation in order that the licen-see or purchaser may cause the deviation to be evaluated unless the deviation nas been corrected, and-assure that a director or responsible officer subject to the regulations of this part or a designated person notifies the Commission when he obtains information reasonably indicating a failure to comply with any regulation or defect affecting a basic component.

Contrary to the above, numerous examples were revealed where Automatic Switch Company (ASCO) has knowledge of deviations, omissions, and/or potential generic problems and failed to either evaluate them for potential reporta-bility or inform the licensee so they could evaluate them for deportability.

Examples are as follows (88-01-01):

(1) Engineering Investigation Report (EI) 3584, dated October 8,1981 -

Inadequate end margin on a nuclear line, Class H, electrical coil and use of silicone

[ appears rubber generic to by the coil manufacturer to mask the problem 220-339 nuclear line that these coils which were t, originally procured commercial grade may have this problem];

(2) EI 3850, dated October 8,1985 - Valves not shifting adequately in the ASCO nuclear (NP) 8316 series [ Hope Creek];

(3) EI 3871, dated February 26, 1986 - Insufficient differential pressures across certain NP 8316 solenoid valves - valves stuck open [ Comanche Peak];  !

(4) EI 3916, dated July 24, 1986 - Pilot orifice drilled in wrong location on HP 8321 solenoid series. Results in hesitation - four, in 17 NP 8321 valves; (5) El 3643, dated May 18,1982 - A 5/64-inch hole was being drilled in wrong location - Involves the 8321 solenoid body that is used on the nuclear NP series; (6) EI 3825, dated March 19, 1985 - Diaphragms interchanged between ports causing sluggish shifting in NP 8316 line; h
