APPENDIX A NOTICE OF VIOLATION TU Electric Dockets:

50-445/88-05 50-446/88-04 Comancho Peak Steam Electric Station Permits:

CPPR-126 Units 1 and 2, Glen Rose, Texas CPPR-127 During an NRC inspection conducted on January 5 through February 2, 1988, a violation of NRC requirements was identified.

accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1987), the violation is listed below:

documented instructions, procedures, or drawings.

Comanche Peak Engineering Specification 2323-MS-85, t

Revision 5, dated September 15, 1987, Appendix I, paragraph 6 requires that depressions produced by grinding will not exceed 1/32" for 18 gauge metal and thicker.

Sheet metal thinner than 18 gauge must be evaluated by an engineer.

Contrary to the above:

On Duct Segment B-1-658-015, the NRC inspector observed a i

depression that appeared to have been caused by grinding.

An Ultrasonic Digital Thickness Report steted that this sece. ion of duct was fabricated from 16 gauge material which is thicker than-18 gauge material.

This depression exceeded 1/32" in depth which is a minimna wall violation.

The NRC inspector could not find any t

indication that the minimum wall violation in question i

had been evaluated by engineering or QC.

This is a Severity Level IV violation (Supplement II)

required to submit a written statement or explanation to the U.

Nuclear Regulatory Commission, ATTN: Document Control Desk, 1

Washington, DC 20555 with a copy to the Assistant Director for Inspection Programs, Comanche Peak Project Division, Office of Special Projects, within 30 days of the date of the letter transmitting this Notice.

This reply should be clearly marked as a "Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation if admitted, (2) the corrective steps that have been taken and the results achieved, i

(3) the corrective steps that will be taken to avoid further l

violations, and (4) the date when full compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked or why such other action as may be proper should not be taken.
