Revision 5, dated September 15, 1987, Appendix I, paragraph 6 requires that depressions produced by grinding will not exceed
1/32" for 18 gauge metal and thicker. Sheet metal thinner
than 18 gauge must be evaluated by an engineer. l

' t Contrary to the above:

1. On Duct Segment B-1-658-015, the NRC inspector observed a i depression that appeared to have been caused by grinding.

An Ultrasonic Digital Thickness Report steted that this sece. ion of duct was fabricated from 16 gauge material ,

which is thicker than-18 gauge material. This depression  ;

exceeded 1/32" in depth which is a minimna wall  ;

violation. The NRC inspector could not find any t indication that the minimum wall violation in question i had been evaluated by engineering or QC.

This is a Severity Level IV violation (Supplement II)
