APPENDIX A Westinghouse Electric Corporation Docket No. 99900005/86-02 NOTICE OF NONCONFORMANCE During an inspection conducted October 6-9, 1986, fabrication and testing activities and the implementation of the Quality Assurance (QA) Program at the Westinghouse Nuclear Fuel Division Facility in Columbia, South Carolina were reviewed. The CA program requirements are discussed in the Westinghouse Electric Corporation Quality Assurance Plan WCAP 8370, Revision 10A and WCAP 7800, Revision 6A, dated August, 1984. This program includes activities associated with the fabrication of fuel assembly components manufactured at the Columbia facility. Section 17.1.5 of WCAP 8370/7800, Revision 10A/6A, dated August 1984, requires, in part, that activities affecting quality are accomplished in accordance with documented instructions, procedures, or drawings. As discussed below, several examples were identified in which procedures were not adequately implemented.

Based on the results of this inspection, it appears that certain activities at the Columbia facility were not conducted in accordance with commitments made to the NRC. These items are set forth below and have been identified and classified in accordance with the NRC Enforcement Policy,10 CFR Part 2, Appendix C.

1. Section 7.1 of Mechanical Operating Procedure (M0P)-730502, " Fuel Assembly Cleaning - General," Revision 13, dated August 21, 1986, recuires, in part, that the wash / rinse tank temperatures be verified at the following operating temperatures: Tank 1 - 145* to 175 F, Tank 2 - 165 to 180 F, Tank 3 -

145 to 195 F.

Contrary to the above, the wash / rinse tanks were observed to Tank be at 1the following temperatures while a fuel assembly was in Tank 2:

142 F, Tank 2 - 162*F, Tank 3 - 122 F. (86-02-01)

2. Section 7.0 of Chemical Operating Procedure (COP)-814741, Revision 4, dated March 21, 1986, requires, in part, that the revolution counter be set at 160 for blending a container of uranium dioxide.

Contrary to the above, the operator set the revolution counter at 10 for blending a container of uranium dioxide. (86-02-02)

3. Sections 4.2 and 7.1 of COP-814742, Revision 5, dated August 26, 1986, require, in part, that when taking samples from a bulk container for quality control analysis, the sample hood ventilation velocity should be monitored weekly.

Contrary to the above, the sample hood ventilation velocity was being monitored monthly instead of weekly. (86-02-03)
