NOTICE OF NONCONFORMANCE 6

During an inspection conducted July 6-10, 1987, the implementation of the Quality Assurance (QA) Program at the PSI facility in North Stonington, Connecticut was reviewed with respect to the fabrication of nuclear butterfly valves. The applicable QA Program requirements are 10 CFR Part 50, Appendix B,

. and PSI's Quality Assurance Program Manual (QAPM), Revision 0, dated December 19, 1986. Based on the results of this inspection, it appears that certain activities were not conducted in accordance with these requirements.

The following nonconformances were identified:

Criterion IX of. Appendix B to 10 CFR Part 50 requires, in part, that

measures be established to assure that special processes including nondestructive examination (NDE) are controlled and accomplished by qualified personnel using qualified procedures.

Section 12.30 of the QAPM requires that NDE procedures including those of vendors be approved by the Level III examiner and also be reviewed and approved by the Manager of Quality, Section 12.40 of the QAPM states, in part, "NDE services may be subcontracted to qualified vendors using qualified procedures approved by Pu i-Seal."

Contrary to the above, it was noted that three NDE procedurer - Nos.

135-HAR-UT-29, 23.A.200-1986, and 23.A.031-1986 - used by Magnaflux/MQS Inspection in February 1984 and June 1987 were not reviewed by PSI.

2. Criterion IX of Appendix B to 10 CFR Part 50 requires, in part, that measures be established to assure that special processes including HDE are -

controlled and accomplished by qualified personnel using qualified procedures.

Section 12.40C of the QAPM states, in part, " Subcontractors' hDE personnel qualification records, as defined in SNT-TC-1A...shall be maintained by the Quality Assurance Specialist...."

Section 9.6.1 of SNT-TC-1A requires that qualification records of certified individuals include, among other things, the following: educational i background and experience, results of eye examinations, and evidence of successful completion of examinations.

Contrary to the above, it was noted that PSI did not have the qualifica-tion records for four individuals from Magnaflux/MQS Inspechon who had perforraed NDE In February 1984, February 1986, and June 1987. (87-01-03) 8709100405 870909 PDR GA999 ENVPOSIS.

l l . -3. Subsection NCA-4143 of Section III of the ASME Code states, "To'the extent necessary, procurement documents shall require suppliers to provide a Quality Assurance Program consistent with the applicable requirements of this~Section." -

Criterion IV of Appendix B to 10 CFR Part 50 states, in part, "To the extent necessary, procurement documents shall require contractors or subcontractors to provide a quality assurance program consistent with the pertinent provisions of this appendix."

Contrary to the above, a review of P0s revealed that quality requirements outlined in Section III, of the ASME Code were not passed on to vendors for the following P0s: 11658 and 11659 (March 2, 1987) to Airmotive Calibration Corporation; 3689 (April 24, 1979), 3735 (June 4, 1979) and 4851 (July 30, 1979) to Cambridge Plating; and 3692 (April 27, 1979) to Fountain Plating. 4. Subsection NCA-4134.5 of Section III of the ASME Code and Criterion V of Appendix B to 10 CFR Part 50 state, " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a

Contrary to the above, written instructions or procedures did not exist
