" ' ', APPENDIX B Rockwell Engineering Company Docket No. 99900836/87-01 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on February 19, 1987, it appears that certain of your activities were not conducted in accordance with NRC requirements.

A. Criterion IV of Appendix B to 10 CFR Part 50 states, in part, "...to the extent necessary, procurement documents shall require contractors or subcontractors to provide a quality assurance program consistent with the pertinent provisions of this appendix."

Contrary to the above and to the RE QA Manual, Revision 9, dated May 3, 1982, the requirements of 10 CFR 50, Appendix B, were not passed on to the RE subtier vendors Inland Steel. Empire Galvanizing, Weld Star,

and Central Steel and Wire while fulfilling P0-ET 16183 for VEPC0 for the new service water spray array system supports. The VEPC0 PO-ET 16183 imposed the requirer.ents of 10 CFR Part 21 and 10 CFR 50, Appendix B on RE. (87-01-04)

B. Criterion IX of Appendix B to 10 CFR Part 50 states, in part, " Measures shall be established to assure that special processes, including...

nondestructive testing, are... accomplished by qualified personnel...in accordance with applicable.... standards,...and other special requirements."

Contrary to the above and to the RE procedure " Qualification, Training and Testing of Ou'ality Assurance Inspector," RECO 1900-1, Revision 3, and the recommended practices of SNT-TC-1A, two QA inspectors with NDE responsibilities were certified without record of a vision examination.

There was no ob,iective evidence that a recertification examination was administered and taken by one of the two 0A inspectors. (87-01-05)

C. Criterion XV of Appendix B to 10 CFR Part 50 states, in part, " Measures shall be established to control materials, parts, or components which do not conform to requirements in order to prevent their inadvertent use or installation. These measures shall include.... procedures for....

segregation....."

Contrary to the above and to the RE 0A Manual, Revision 9, dated May 3, 1982, at the time of the inspection RE had no procedures to address  ;

provisions for the segregation of nonconforming materials. (87-01-06) i
