APPENDIX A NOTICE OF NONCONFORE NCE GE Nuclear Energy (GE NE) Quality Assurance Program Topical Report,

.NED0-11209-04A, Section 7 " Control of Purchased Material, Equipment and Services," states that procedures and practices are established and documented to provide assurance that. purchased items and services, whether purchased directly or through subcontractors, conform to procurement document requirements._ Documentation of such requirements is originated and maintained by the cognizant QA organizations.

GE Nuclear Services Procedure N."-50-01, Revision 2, dated September 15, 1987,

" Procurement Initiation & Controi," states in Section 3,6.6(b) that suppliers are to be audited annually to their quality program commitments.

Contrary to the above, GE NE purchased commercial grade electrical components, including molded case circuit breakers, from the GE supply Company, Santa Clara, California, without the supplier being approved and included on GE NE's Approved Supplier List. GE NE requested and accepted documentation from GE Supply which certified that the material supplied was undamaged and in

' altered, tampered with, or modified from its original configuration. This certification was relied upon by GE NE without the benefit of supplier audit to verify its validity (88-02-01).
