h l i Septcabsr 20, 1976 PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE--PNO-76-158 '

( This preliminary notification constitutes EARLY notice of an event of POSSIBLE safety or public interest significance. The information l presented is as initially received without verification or evaluation and is basically all that is known by IE staff on this date.

FACILITY: American Electric Power Company, D. C. Cook Unit 2 (Docket No. 50-316) Bridgman, Michigan

SUBJECT:

INDUSTRIAL FATALITY AT D. C. COOK UNIT 2 American Electric Pcwer Company informed Region III, by telephone on September 17, 1976, that an industrial accident had resulted in the death by asphyxiation of one construction worker and hospitalization of two other workers. The two other workers remain in critical condition at Mercy llospital in St. Joseph, Michigan.

The accident occurred in the recirculation sump in the lower elevation of the D. C. Cook Unit 2 containment building. The three worlanen were performing welding operations when they were apparently overcome by a collection of the welding cover gas (argon) shortly after 5 p.m., EST.

Four members of the rescue team also required treatment but were released from the hospital.

The licensee notified OSHA and State of Michigan of the accident and made a press release.

Unit 2 is under construction and is approximately 70% completed. No radioactivity was involved. Fuel has not been received for Unit 2.

Region III Glen Ellyn, Illinois was notified at 6:50 p.m., EST, by J Argonne National Laboratory of the incident through an inquiry which was initiated by Mercy Hospital to Argenne Lab regarding toxicity of argon gas. The resident inspector at Stevensville, Michigan then obtained details of the accident at 7:15 p.m., EST. The PN reflects updated information as of September 20 - 3:00 a.m.
