PRELIMINARY NOTIFICATION OF EVENT OR UNUSUAL OCCURRENCE PNO-III-90:49 Date Jul,< 13, 1990

This p'reliminary notification constitutes EARLY notice of events of POSSIBLE safety or public

~ interest significance. The information is as initially received without verif'caticn or evaluation, and is basically all that is known by the Region III staff on this date.

Cook, Unit 2 X Unusual Event Stevensville, MI 49127 ~ ~ Alert Site Area Emergency Docket No. 50-316 ~ General Emergency Not Applicable S:bj:ct: ONE PERSON KILLED, THREE INJURED IN ELECTRICAL ACCIDENT One person was killed and three others injured when the fatally injured worker touched a live contact in a 4 kilovolt cabinet at 1:10 p.m. on July 13, 1990. Unit 2 has been shut down for refueling since June 30, 1990. Unit I was and is operating normally at 100% power.

. A circuit breaker had been previously removed and the worker, a contract employee, stuck his hand into the cabinet, touching the live contact. There was also a sme.11 fire, but it was extinguished within 5 minutes by the plant fire brigade. It is not known why the worker stuck his hand in the cabinet.

The reactor was being cooled by the residual heat removal system, which was not affected by the incident.

Ambulances were called to the site, and the injured were taken to area hospitals. The c;ndition of the injured workers is not known, although two of them were described as being 2

seriously" injured.

The licensee declared an Unusual Event under its emergency plan and subsequently terminated it. The_ resident inspectors are monitoring the licensee's investigation of the incident.

.The information in this Preliminary Notification has been reviewed with licensee management.

The State of Michigan will be notified.

There has been local news media interest in the incident.

The licensee notified the Headquarters Operations Center of this incident at 1:25 p.m.
