Executive Summary The Connecticut Yankee Plant Design Change Task Group (CY PDCTG) was established in January 1985 in response to an NRC order dated December 13, 1984. This is the first milestone report of the CY PDCTG.

This report provides the results of the first phase in the review of CY plant design changes for potential safety concerns. This phase consisted of screening all plant design changes except those completed via work permits to determine which changes would require a detailed review.

The task group reviewed all Plant Design Change Requests (PDCR's) that were approved during the period January 1,1979 through December 31,1984, except for those that were subsequently cancelled. Of the 355 PDCR's, in this category, 35 (10%) were determined to require a detailed review. These PDCR's are listed in Table 1.

I The task group also screened 12 design changes involving jumpers, lifted leads, or bypasses that were approved prior to January 1,1985 and were still in effect as of April 15,1985. None of these were determined to require detailed review. .

Plant design changes performed through Work Permits or Automated Work Orders without a corresponding PDCR were reviewed by a contractor (Impell) and will be addressed in a separate report. The scheduled completion of the screening of these documents is June 1,1985.

The screening review confirmed an improvement in the PDCR packages with time. A significantly higher percentage of the 1979 80 PDCR's were selected for detailed evaluation, primarily because of a lack of thoroughness during that time period.

Approximately half of the PDCR's selected for detailed evaluation were chosen because of potential seismic qualification concerns. Some are simple concerns (e.g. addition of components to main control board) whereas others question the seismic basis for an entire system (e.g. service water and boric acid).

No immediate safety concerns or need for immediate corrective action have been identified during the screening process.

1. Introduction The Connecticut Yankee Plant Design Change Task Group (CY PDCTG) was charged with the responsibility of evaluating all plant design changes approved since January 1,1979 to determine if any public safety concerns were introduced. The first step in this process was a preliminary evaluation of al design changes to determine those which required a more detailed review because of their potential public safety significance. This preliminary evaluation was termed " Screening".

Plant design changes were made in one of three ways. The primary means was through the use of a Plant Design Change Request (PDCR). There were 355 PDCR's approved during the time period January 1,1979 to December 31,1984 which were not subsequently cancelled.

1 Another method of making a design change which has its own control method separate from PDCR's is through the use of jumpers, lifted leads and bypasses. These are temporary changes and hence there were only 12 which were approved prior to January 1,1985 that were stillin effect as of April 15,1985.

It was determined that some design changes were made through the use of Work Permits and Automated Work Orders without approval under a PDCR.

Because of this determination, the CY PDCTG decided that all Work Permits / Orders during the 1979 through 1984 time period would have to be screened for design changes of potential safety significance.

Since the number of Work Permits / Orders during the period January 1,1979 through December 31,1984 numbered approximately

, 20,000, the screening was performed by a contractor (Impell). The results of this screening will be provided in a separate report by the contractor.

Screening was initiate'd on March 4,1985 and was completed on May 6,1985. It was the primary activity of the Task Group during this time period. All 355 PDCR's and 12 jumpers were screened by the Task

. Group and are the subject of this report.

II. Method Screening of PDCR's and jumpers was performed through the use of CY PDCTG Procedure 1.02 (Attachment 1). The basic review logistics incorporated by this procedure included the following:

1. A lead screening reviewer was assigned from the group for each design change. _The lead reviewer would obtain the design change package as well as any other material necessary to adequately evaluate the change. The lead reviewer would then evaluate the change against the criteria established for determining safety significance and document the evaluation.
2. The lead screening reviewer's evaluation would then be routed to each individual in the PDCTG for independent evaluation. This was an important step as it gave each individual the time to review the package in detail independently rather than just discussing the change in a group forum where one might have limited time to consider the change in detail. Any questions or comments an individual may have were recorded on the form for discussion at a group meeting.
3. Af ter each individual had reviewed the change, the lead screening reviewer would present the change to the group for discussion and final determination. The group discussion was important since these discussions provided new insights and new questions which may have
been overlooked during the independent reviews. Of ten, PDCR's

were put on hold to resolve outstanding concerns noted during the group meeting. A subsequent group meeting would be held to make a final determination.

4. Concurrence of all 6 members was required for a "No Further Evaluation" determination. Any one member could dictate a detailed review.

Three screening criteria were established in the procedure. However, these criteria were generally worded to avoid a regimented and perhaps limited thought process. The criteria required an evaluation of the design change against the following three general concerns:

1. Potential loss of a boundary designed to contain radioactivity (cladding, pressure boundary, containment). If the change itself, or a credible failure of the change could result in an increased probability of failure or a previously unanalyzed failure mode of a boundary then it would require detailed evaluation. Additionally, a change which could increase the consequences of a previously analyzed failure required review. Secondary effects (e.g. flooding, falls, etc.) were considered in this evaluation and hence even changes to non-safety-related systems could have an effect on safety systems or boundaries.
2. Thoroughness of the change package for safety related changes. If the change affected safety related equipment, but the package did e , - - - - a ,,

not appear thorough (e.g. did not address seismic issues, involved 3 specific changes but only 2 were addressed in the design review and safety evaluations) then it would require a detailed evaluation.

3. Adequacy of the safety evaluation. The written safety evaluation (and backup documentation) was reviewed against the complexity and safety significance of the change. If inadequate (e.g. safety i

significant change involving multi-disciplines but only the mechanical aspects were evaluated) the change may require detailed evaluation.

A form documenting the lead screening reviewers evaluation, independent review questions and comments, a summary of the group discussion, and the bases for 3 the final determination was completed and filed with each design package. For those design changes which were determined to require additional review, concerns which arose during the screening prc/ ess were documented to ensure their resolution during the detailed evaluation.

III. Results

- The PDCRs approved between January 1979 and December 1984 have been evaluated under the screening process. There were 413 candidates, ranging from PDCR No. 282 through PDCR No. 694. Of these, 58 were cancelled

.or are still not PORC approved. Thus, 355 have been evaluated during screening. Of these, 35 have been selected for detailed evaluation. This corresponds to 10% of the P1)CR's evaluated in screening. A listing of the determination (Yes or No) for detailed evaluation for each PDCR is given in Attachment 2.

Table i provides a separate listing of the 35 selected for detailed review.

In addition, all jumpers / bypasses / lifted leads initiated prior to January 1,1985 and still in effect as of April 15,1985 were screened for detailed evaluations. The twelve items screened are shown in Attachment

3. No safety concerns were identified for these jumpers and none were identified for detailed evaluation.

IV. Conclusions In reviewing the PDCRs identified for detailed evaluations, some trends have been noted. As expected, approximately half of these PDCR's were approved prior to 1981. This tends to confirm the improvement in the PDCR packages as experience, training and improved procedures have been 4

developed. This is shown in Figure 1. Nine PDCR's approved in 1984 have been selected for detailed evaluations. This does not represent a decline in the quality of the design process, but reflects the increase in the number of PDCR's processed. In addition, several of these PDCR's were selected i because of questions on seismic classification, QA qualification and testing I

requirements for systems already in place prior to the change and unaffected by the change itself. This is further supported by the increase in thoroughness of the PDCR packages for the changes approved in 1983 and 1984.

f The questions raised about the PDCR's that lead to the requirement for detailed review have been categorized as shown in Table 2. By far the most predominant issue to be addressed is seismic qualification of equipment, having been identified as a question on approximately half of the PDCR's. Some of them are simple issues such as the addition of a component to the main control board, whereas others involve the seismic l

design of an entire system such as service water or boric acid.

Of these questions raised during screening, none have been determined to require immediate corrective action. Identified concerns will be addressed In the detailed evaluation phase of this project.

, 1.0 PURPOSE This procedure provides the method to be used to perform an initial i evaluation of plant design changes and determine which ones require further detailed evaluation.

2.0 APPLICABILITY 4

This procedure applies to all Task Group Members. The procedure applies to all plant design changes requiring screening per Reference 3.1.

5.3 PDC Task Group Chairman Responsible for designating the Lead Screening Reviewer for each plant design change.

5.4 Lead Screening Reviewer Responsible for performing an initial assessment of a design change and presenting the change to the remainder of the Task Group.

6.0 INSTRUCTIONS 6.1 A Lead Screening Reviewer shall be designated by the Task Group Chairman for each plant design change and the Reviewer shall compile the design change package. The Lead Screening Reviewer shall not have been involved in the engineering, design, or saf ety evaluation of the change.

6. 2 A Screening Review Sheet (Figure 7.1) shall be completed for each plant design change as follows:

,' 6.2.1 The Lead Screening Reviewer shall enter the PDC number, the date of approval and the title of the change. He shall circle his name in Section I.B.

6.2.2 The Lead Screening Reviewer shall then answer each of the screening criteria,Section II, with either a "yes, "no" or

" uncertain (?)" answer. He shall also enter the basis for each answer. If he has any questions or considerations for group discussion he should enter them in Section ll!. A.

6.2.3 The Lead Screening Reviewer shall then route the Review Sheet along with the Plant Design Change Package to each Task Group Member as listed in Section I.B.

6.2.4 Each individual shall review the change against the screening criteria in Section II. If desired, he may modif y the basis as developed by the Lead Screening Reviewer in Section 11 or enter questions or . considerations for group discussion in Section Ill.A. He shall then cross his name off in Section I.B and route to the next individual. All six members shall review the package.

( 6.2.5 The last individual shall return the Review Sheet and package to the Lead Screening Reviewer to schedule a presentation to the Task Group.

6.2.6 A Task Group Meeting will be held to make a final determination. A minimum.of five Task Group Members shall be present.

6.2.6.1 The Lead Screening Reviewer shall present the package to the Group and complete the information on the top of Section III.

6.2.6.2 Any questions or considerations listed in Section Ill.A from the individual reviews should be discussed.

6.2.6.3 A summary of the group discussions shall be 4

recorded in Section !!!.B.

6.2.6.4 The Final Group determination shall be entered in Section Ill.C along with the basis. If appropriate, reference to the written Bases in Section II is acceptable. All members must concur for a "no" determination. .

r 6.2.6.5 If a detailed evaluation is required, and possible

!A considerations for that evaluation were developed during the screening review process, those thoughts may be entered in Section 111.D for f uture use.

6.2.7 Final signoff by all six members sha!! be made in Section IV indicating concurrence with the final determination. If a member was not present at the group meeting and does not concur with the group decision, he may request another group

6.3 The Screening Review Sheet shall be filed with the design change package.

6.4 The Task Group Chairman shall maintain an updated log of all design changes in the screening process. The log shall list as a minimum, design change number and title, Lead Screening Reviewer, and the screening determination.

6.5 No further action is required for a "no" determination. Those i changes with a "yes" determination shall be evaluated in accordance with Reference 3.2.
