ABSTRACT OF EXAMINATIONS This report contains a summary of the examination results for the inservice inspection (ISI) conducted at the Connecticut Yankee Atomic Power Company (CYAPCO) Haddam Neck Plant during Refuel Outage 19. Included in this report are required ASME Code data report forms for items examined and associated repairs and replacements that have been completed since the last Nuclear Inservice Inspection Summary (NIS) Report submitted in July 1995.

The ISI was conducted during the period of July 25,1996 to December 4,1996 and was the result of an unplanned Refueling Outage (RFO-19). A total of 286 items were examined by Vendor, Northeast Utilities (NU) and CYAPCO personnel. These examinations were performed to meet the requirements of the CYAPCO ISI Program Plan.

The ISI Program Plan has been written and updated to include requirements specified in the CYAPCO Plant Technical Specifications, Code of Federal Regulations 10 CFR 50.55a, certain Nuclear Regulatory Commission (NRC) approved ASME Code Cases, NRC granted relief requests, various other regulatory and engineering commitments, and the ASME Boiler and Pressure Vessel Code,Section XI,1983 Edition including the Summer 1983 Addenda.

All required examination records and documentation generated during the performance of the inserr inspections are maintained at the Connecticut Yankee Haddam Neck Plant for the service lifetime of the plant.

INTRODUCTION The summary ofISI examination results presented in this report includes 286 items. As applicable the ISI program plan has been written and updated to comply with the requirements of the ASME Code Section XI,1983 Edition including the Summer 1983 Addenda, CYAPCO Plant Technical Specification Section 4.0.5, NRC approved ASME Code Cases N-278, N-408-2, N-416, N-426, N-436, N-437, N-460, N-481, N-490, N-491 and N-498, NRC granted relief requests, and various other regulatory and engineering commitments that are identified as augmented examination items in the plan.

The examination records and documentation provided for all Code Credit items have been reviewed by the Authorized Nuclear Inservice Inspector (ANII) employed with the Hartford Steam Boiler Inspection and Insurance Company.

The ISI was conducted from July 25,1996 to December 4,1996 and included 580 scheduled examination items. These items were scheduled as part of the normal outage ISI program plan requirements. The ISI program plan was not completed as a result of the Board of Directors decision to cease operation of the facility.

Supplemental reports are included in Section 14 " Class 1 NIS-2 Reports" and Section 15 " Class 2 NIS-2 Reports" for applicable NIS-2's that were not completed for submittal in the 1995 repon.

1.' ASME SECTION XI INSERVICE INSPECTION Nondestructive examinations m.re performed in accordance with Section XI of the ASME Boiler and Pressure Vessel Code,1983 Edition, Summer 1983 Addenda. The examinations completed were in accordance with scheduling requirements for the Third Period of the Third 10-year Interval.

Manual nondestructive examinations were contracted from ABB Amdata Inc.

Connecticut Yankee Atomic Power Company (CYAPCO), and Northeast Nuclear Energy Company (NNECO) personnel participated in various nondestructive examinations in support of all inservice inspections.

2. EXPLANATION OF RESULTS 1

Listing of items / components examined during the inservice inspections covered by this report are presented by Examination Categories, in a column format, under each of the Examination Results Sections 8 - 12. These sections are separated by applicable ASME Code Classifications 1,2, and 3, Subsection IWF Component Supports, and Augmented Examinations. The columns under the Examination Categories in these sections are identified as Component, Results, and Comments. Any conditions noted about the results of examinations performed for inservice inspections are described in the Comments column. If additional details are required to describe these conditions specific reference is made to items in this section.

Under the results column several terms are used to define the disposition of examination results. These terms are related to the conditions noted for each item as follows:

Supplemental MT was performed and an indication of approximately 10" was observed and noted that it extended beyond the prepared area. Further paint was removed and the area was re-examined by MT the total length of the indication was 29" with the indication extending to the wall penetration. The area outside the penetration was prepped and MT examined. The indication extended an additional 10" to the next weld on the piping run where it ended. Supplemental UT depth sizing was performed and the indication was deter.nined to have a maximum depth of.1". The piping was cut out and sent to the Metallurgy lab in Berlin where destructive testing determined the indication was a fabrication defect present in the piping. In the interim while waiting for the laboratory results, a conservative decision was made to examine all integrally welded attachments on the service water system for supply and return piping for the spent fuel pool cooling heat exchangers.

CORRECTIVE MEASURES FOR COMPONENT SUPPORTS OF SECTION 10 Item 1 20312-07-WS-2028-W The piping was cut out and sent to the Metallurgy lab in Berlin where destructive testing determined the indication was a fabrication defect present in the piping. All integrally welded attachments on the service water system for supply and return piping for the spent fuel pool cooling heat exchangers were subsequently examined by MT. The pipirig was replaced and the system restored with VT performed for baseline examination.
