i May 1, 1984 Region 1, USNRC Office of Inspection & Enforcement 631 Park Avenue King of Prussia, Pa. 19406 Att: Director of Inspection & Enforcement In accordance with 10CFR19.16 I hereby request an inspection by your office of incidences which occurred during March 14, 1984 through March 27,1984 concerning a maintenance job involving the finding and repairing of a leak in the Radwaste Overboard Discharge Pipe at the Oyster Creek Nuclear Generating Station.

Af ter unsuccessfully seeking resolution to the problem I have determined that the following violations did occur and the same type of violations could occur again:

1.10CFR19.12 - Workers not informed of the radiation hazards involved with the job.

2.10CFR20.201(b) - No air sample run while torch cutting of contaminated pipe was in progress t_

3. Oyster Creek Procedure No. 915.12, sec tion 3.1.5 - Prerequisites section of procedure calls for a Radiation Work Permit to be issued when there is an unknown condition. Nor was a special RWP issued for an emergency condition as called for under section 3.2.4 of the same procedure.
4. Oyster Creek Procedure No. 915.10, section 5.3.4 - The investigation of the incident should have been completed within two weeks of it's occurrence and has not been completed as yet.

Surveys taken during the period of work on this job by the Radiation Control Department and the Chemistry Department indicated contamination. However, work progressed without a Radiation Work Permit being issued.
