SULJECT: Grand Gulf Nuclear Station Units 1 and 2 Docket Nos. 50-416/417 File 0260/15525/15526 PRD-80/49, Interim Report #3, Guyon Coupling Linear Indications AECM-81/119 On August 11, 1980, Mississippi Power & Light Company notified Mr. M.

Hunt, of your office, of a Potentially Reportable Deficiency (PRD) at the Grand Gulf Nuclear Station (GGNS) construction site. The deficiency concerns linear indications on couplings supplied by Guyon Alloys, Inc. for use in Nuclear Class I and II applications.

Our Architect / Engineer is conducting an investigation to ascertain the extent of the problem and to determine applicability of reportability criteria per 10CFR50.55(e) and 10CFR21.

Interim Report No. 3 is attached detailing the status of the deficiency to date. It is anticipated that a final report can be provided by Au;;n.st 19, 1981.

Attachment to AECM-81/119 Page I of 2 INTERIM REPORT NO. 3 TO PRD-80/49 I. Description of the Deficiercy Linear indications have been found in the f ace and along the body of four nuclear Class 2 and two nuclear Class 1, three-quarter inch (3/4")

couplings manufactured by Alloy Stainless Products Ccepany and supplied by Guyon Alloys, Inc. These couplings all have the same Heat Code Number and Mill Heat Number (Heat Code QE, Heat Number A17130) and are installed in the safety piping systems. It is known at this time that these couplings are installed in the Reactor Pressure Vessel Instrumentation Piping.

II. Resolution of the Deficiency The six couplings described in the " Description of the Deficiency" were determined to be in the sensing lines on the Reactor Pressure Vessel instrumenta*, ion piping. An investigation determined that loss of the sensing lines in which these six couplings were located would result in automatic shutdown of the reactor. However, MP&L has deter =ined that many other similar Guyon coaplings having the same hest number have been shipped to the GGNS site. The Constructor is presently in the process of locating these couplings and performing a non-destructive examination p

of them.

III. Status of Proposed Resolution An investigation is presently underway to deter =ine the full extent of the problem. .To date, a total of 540 QE couplings have been received at Grand Gulf. Of this total,-298 have been-located and documented. Of the 242 remaining, 161 have been located and are awaiting evaluation.

Further investigation is underway to locate the 81' fittings not .yet

-accounted for.

Examination of 171 fittings in stock and 121 fittings installed in Q lines revealed 20 (19. in. stock and l ~ installed) of the f,ittings (Class

2) possessed indications similar to the described condition.

Our Constructor has performed a walkdown. investigation and has B21, identified couplings with subject Heat ~ Codes QE in six systems:

B33, E31, E61, G33 and P44.

The Constructor's Nonconformance Report (NCR) 4966, the subject of:

-Quality Action Request (QAR) F-208, identified 2-Class 1 and four Class 2 couplings.having linear indications.- Class 2 couplings with linear indications were sent to our Architect / Engineer's San Francisco office On the
