Dear Mr. Keppler:

I This letter serves as the Final Report as required by 10CFR50.55(e) concerning i lamination identifled in ASTM A36 gussec place hanger steel material. This was [

first reported by W. J. Racer of the Cleveland Electric Illuminating Company l to J. Nelsler of your office on December 19, 1980, and was followed up by the i Interim Report dated January 15, 1981. ,

This Final Report includes a restacemenc of the description of the deficiency, l the corrective accion taken and overall conclusions.  ;

Descripcion of Defielency [

Some 1/2" x 5" x 8" places used for gussets on Class 1 eleccrical c.able cray I

hangers were found to have linear indications along the edges. An inveselgacion ,

showed that these indicacions were laminacions in the steel plate as received f rom the supplier. The place was cut from ASTM A36 macerial. This condicion l was documenced on L. K. Conscock Nonconformance Report EC-179.  !

Corrective Accion Taken  !

1. All gusset plate material of ASDi A36 held in field stock af ter February 1, [

1980, was visually inspected for edge laminacions by the contractor (L. K. L Comstock) prior to ins tallation. Any place macerial exhibiting laminacion l was not used in any safecy-related hanger fabricacion.

2. Concurrent with icem (1) above, hanger place material was changed via  !

Engineering Change Nocice No. 3862-33-545 from ASTM A36 to ASTM A36 with i supplementary requirement S1 or ASDi A516 grade 70 with supplementary ,

requirement S8. This change was incended to improve place quality and

minimize the potencial for laminacions. Since che depletion of remaining field stock of A36 place material from item (1) above, all material has conformed to che new macerial requirements.

3. For A36 gusset places installed prior to February 1,1530, a random sampling inspeccion program was conducted by L. K. Comscock Qualici Control Inspectors per guidelines as specified by the Project Organiza tion Nucicar Denign Section.

Based upon installation records, L. K. Conscock has identified .:.ac 498 hangers wich A36 gussec places were installed prior to February 1, 1980.

Thirty-chree (33) of chose gusset places were chosen on a random sample basis and inspec ced pe r che follovin6:

a. The selected gussecs were ground on both 5" long place edges to remove existing painc. Exposed edges were visually inspected in accordance with the fn11owing acceptance criteria:
1. Laminar type edge discontinuities were not to exceed 1" in length.
2. Discontinuicies were considered to be continuous when separated by a distance less chan the length of the smaller of the two adjacent discontinuities.
b. If the above acceptance criteria were violated, che place edges were re-ground and re-inspected per che above criteria, with the limitacion that the cocal amount of place material to be removed by grinding should not exceed 1/8" on each edge. If the acceptance criteria were still not satisfied af ter grinding 1/8", che gussec place was to be reported to the Project Organizacion as a violacion.

Results of the above inspeccion program have shown that all of the selected gussec places have me t the specified acceptance criteria. All inspeccions have been docu-menced by L. K. Comstock QC inspectors.

Analysis of Safecy Implicacion

1. '4ork installed prior co February 1,1980:

Since che gusset places selected for inspeccion are representative of all places installed prior to February 1,1980 and rul acceptance criteria vio-lacions were found, ic is concludc5 that work installed previous to this date is acceptable "as is". The originally documenced deficiency (L. K.

Comscock Nonconformance Report EC-179) involved cwo grossly laminated places which were pro 1 ably caused by improper billet cropping at the steel mill.

Per disposicio t of NR No. EC-179, these two plates were removed and replaced.

2. Jork installed after February 1, 1980:

As mencioned earlier, all remaining field stock of A36 gusset place was visually inspected for edge laminations. This insures no laminated A36 places have been used in hanger fabrication af *sr this date. Also, the change to different macerial types (ASTM A36 a ad ASTM A516, grade 70 with supplemencary requriements S1 and 3 8, respectively) should insure improved place quality. In addition, L. K. Comstock is doing on-going random in-speccion of this material for edge laminations. Material with laminacions will not be used in any hanger fabricacion.

l Conclusion f

In summary, chis report concludes chac the originally documenced deficiency (NR {

No. EC-179) was an isolated case which was routinely identified because of the  !

gross nature of the laminacions. Appropriate corrective accion has been caken [

co correct chac specific deficiency as well as to insure previously inacalled i I

and fuctire hanger installations are free of significanc place laminacions.
