Report No. 50-219/79/27-lP IDENTIFICATION Discovery of four seismic restraints for the 6 inch core spray 0F OCCURRENCE: test line, which were either in positions other than required by original design criteria or had failed.

This event is considered to be a reportable occurrence as defined in the Technical Specifications, paragraph 6.9.2,a,9.

C0NDITIONS PRIOR X Steady State Power Routine Shutdown TO OCCURRENCE: Hot Standby Operation Cold Shutdown Load Changes During Refueling Shutdown Routine Power Operation Routine Startup Other (Specify)

Operation Power: Core 1909.16 KWt Electric 639 KJe Flow: Recirculating 15.2 x 104 gpm Feedwater 7.143 x 106 lb/hr Stack Gas Activity: 27,900 uci/sec DESCRIPTION On Tuesday, August 7, 1979, during an inspection of seismic 0F OCCURRENCE: restraints associated with Core Spray System II, it was determined that two seismic restraints' (NZ-2-R5 and R9) clamps were not in their as-built position. Additionally, one restraint, NZ-2-R7, was found to have its wall attachment bolts sheared, rendering this restraint inoperable. The structural configuration of the fourth restraint was found to have been altered by some mechanism '

not yet determined. All of the restraints are connected.to the 6 inch test line for Core Spray System II.

The cause of the discrepancies and failures is under investigation.

ANALYSIS OF Until further investigation is performed, Core Spray System II OCCURRENCE: has been considered inoperable, because of the number and location of the restraints affected.

CORRECTIVE Work is underway to restore the restraints to their as-built ACTION: condition. Until the discrepancies have been corrected, Core Spray System II will be considered inoperable and Core Spray System I will be on an accelerated surveillance schedule as per the Technical Specifications, paragraph 3.4,A,3.
