l Occurrence Date: 1/13/91 1

l Identifiggtion of occurrence )

i Inoperable condenser bay sprinkler system #2 was not rentored to operable '

status within fourteen (14) days ao required by Technical Specification 3.12.c.3.

Deecription of occurrence:

one of the fire protection automatic oprinklers on oprinkler systems #2 opened. Thero was no fire present to cause operation of the sprinkler. To stop the water discharge, the manual centrol valve was closed.

Analysis of occurrenqm The 165'F temperature rating of the sprinklers originally installed can be

' exceeded when a steam leak occuro in the condonner bay. The cause of thia

-occurrence is attributed to a steam leak in the vicinity of the oprinkler head.

gorrective Action:

The requiromonte of Technical Specification 3.12.C.2 regarding fire watches during periodo of inoperability woro estab11 shod and are being implemented. The extended period of inoperability is necompary due to the

.condonner bay being a locked high radiation area. The 165'F rating oprinklers are being replaced by 212'F rated sprinklera as failuron of this nature occur. The specific eprinkler which opened in this case will be replaced during the 13R refueling outage. This will allow rostoration of sprinkler system #2 to full automatic operation.
