when both containanet spray pump compartament doors were discovered open on a routine tour of the Reactor Building, and paragraph 4.4.C.3 when required surveillance to verify door closure after entry failed to be perfonned.

DESCRIPTION On July 16. 1980 at approximately 4:30 P.M., an operator OF OCCURRENCE:
performing a routine tour of the Reactor Building discovered that both doors to the containment spray pump compartments (northeast and southeast corners of the Reactor Bufiding 9 l

The operator immediately closed the doors and dogged them shut and reported the incident to the Group Shift Supervisor.

1 APPARENT CAUSE Presently unknown.
However. information is being gathered OF OCCURRENCE:
to determine the cause of this occurrence.
ANALYSIS OF The contairement spray cooling system consists of two OCCURRENCE:
independent cooling loops, each capable of removing heat from the primary contairunen. folloedng a loss of coolant accident.
Since both doors were open, contairunent spray systems I and II were considered to be " inoperable" according to the Technical Specifications.
However, both systests would haya performed their designed functions as long as the structural integrity of the torus remained intact.

.. "i potential leak paths develop which could cause water to enter the pump rooms).

CORRECTIVE._.
The pump room doors were closed and dogged shut immediately ACTION:
upon discovery.
Investigations will be performed to determine the cause of the incident and corrective action will be initiated to prevent future occurrences.
