IV. DOCKET NUMBER: 050-254 V. EVENT DESCRIPTION:

On March 8, 1980, while performing Procedure QOS 005-2, Normal Control Room inspection and Shif t Turnover Panel Check, the Unit One B main steam line radiation monitor was found to be reading low. The B main steam line radiation monitor. was ,

reading about 50 millirem per hour while the A, C and D main steam line radiation monitors were reading about 100 millirem  ;

per hour. The low reading on the B main steam line radiation monitor was determined to be the result of the B logarithmic radiation monitor chassis being out of calibration. This  !

would cause the B system to trip at a radiation level greater than or equal to seven times the background radiation level  ;

(Technical Specification 3.1, Table 3 1-3). '

VI. PROBABLE CONSEQUENCES OF THE OCCURRENCE:

The safety implications of this event are minimal. The A, C 7 and D main steam line radiation monitoring systems were operational and would have provided the required trips on a high radiation level.

Vll. CAUSE:

The cause of this occurrence was equipment failure. The B logarithmic radiation monitor chassis electronically drif ted out of cailbration. The logarithmic radlaiton monitor chassis is a General Electric model 194X629G7 monitor, with a range of zero to one million millirem per hour.

l Vill. CORRECTIVE ACTION:

An instrument mechanic was immediately called to recalibrate the B logarithmic radiation monitor chassis. Thc instrument mechanic calibrated and functionally tested the B logarithmic radiation monitor chassis in accordance with procedure QlS 31-1, Main Steam Line Radiation-Log Rad Monitor Chassis - S.T. -

31A. No further corrective actions were deemed necessary-to prevent recurrence. Surveillance procedure QOS 005-2 verified that the other logarithmic radiation monitor chassis were ,

acceptable.
