On June 11, 1990 at 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />, Quad Cities Unit One and Unit Two were in the RUN Hode at 100 percent of rated core thermal power. At this time, the 1/2A Diesel Fire Pump (FP) exteeded the seven day reporting criteria due to its being-taken out of service (005) for maintenance on June 4, 1990. The 00S was needed so the reliability of the pump could be increased during low water level conditions. A '

five foot suction extension was to be added.

The 1/2B Diesel Fire Pump remained in service.during the duration that i;he 1/2A l Diesel- Fire Pump was out of service and redundant fire suppression water was L available through the service water system. Following completion of the addition of the five foot suction extension, the 1/2A Diesel Fire Pump was successfully tested on June 29, 1990 and returned to service.

This report was initiated by Deviation Report D-4-1-90-047 RUN Mode (4) - In this position the reactor system pressure is at or above 825 psig, and the reactor protection system is energized, with APRM protection and'RBM interlocks in service (excluding the 15% high flux = scram).

. B. DESCRIPTION OF EVENT:

On June 11, 1990,1 0und Cities Unit One and Unit Two were in the RUN mode at 100 percent of rated core thermal power.. At 1500 hours0.0174 days <br />0.417 hours <br />0.00248 weeks <br />5.7075e-4 months <br />, the seven day reporting criteria for an inoperable fire pump (P) (KP] was. exceeded for the 1/2A Diesel Fire Pump (KP, P3. The pump had.been taken out-of-service (00S) on June-4, 1990 at 1650 hours0.0191 days <br />0.458 hours <br />0.00273 weeks <br />6.27825e-4 months <br /> so it-could be disassembled in order to perform maintenance on the pump

,. suction line to increase:the reliability of the pump. 00S 4100-07, Single Fire

-Pump / Hater Supply Outage Report was' initiated.

Technical Specification 3.12.B.2 states that with inoperable fire pumps or associated water supply, restore the inoperable equipment to operable status within-seven days or prepare and: submit a report to the Commission pursuant to

. Specification.6.3.A.1 within-the next 30 days outlining the plans and procedures to be used to provide for the loss of redundancy in this system.

The repairs were completed under work request 064294 on June 26, 1990. During.the-

-course:of performing the fire pump capacity test, procedure QMMS 4100-32,-Diesel Fire Pump Capacity Test, required a revision for valve positions. This procedure; revision coupled with performing the required vibration, capacity.and operability

. . testing delayed the pumps return to service. The 1/2A diesel fire pump was successfully-tested on June 29,.1990 at.2217 hours0.0257 days <br />0.616 hours <br />0.00367 weeks <br />8.435685e-4 months <br /> and returned-to service at.2300

C. APPARENT CAUSE OF EVENT:

This report is submitted to comply w th the requirements of Technical Specification 3.12.B.2, which references Technica' Specification 6.3.A l. Technical Specification 6.3.A.1 requires that the Cofnmission shall be notified and a report submitted pursuant to the requiremt its of Section 50.73 of CFR Part 50.

The cause for the 1/2 A Diesel Fir, P=p b&.g out of service for greater than seven days was because of the req' ired maintenance work on the pump itself. The 1/2 A Diesel Fire Pump takes suchon from the intake bay in the cribhouse. In 1988 a low water level condition occu red, reference Deviation Report (DVR) 4-1-88-014, that resulted in both diesel five pumps being declared inoperable. As part of the corrective actions, a five foot extension was to be added to the suction line of each pump to increase their reliability. This was completed on the 1/2 9 Diesel Fire Pump on April 11, 1988. Due to difficulties in obtaining replacement parts and problems encountered during the part evaluations, the 1/2 A fire pump repairs were delayed.

During the suction extension installation, the pump was found to require numerous repairs. Bearings, impeller and wear rings required replacement, however, the pump had passed the previous monthly testing under QOS 4100-1.

D. SAFETY ANALYSIS OF EVENT:

Technical Specifications allow one of the two diesel fire pumps to be inoperable for seven days. If the time exceeds seven days, a report will be submitted to the NRC within the next 30 days outlining the plans and procedures to be used to provide for the loss of-redundancy in the system.

The 1/2 B Diesel Fire Pump remained operable during the time that the 1/2 A Diesel Fire Pump was out of service. If the 1/2 B Diesel Fire Pump had failed to operate, the service water (BI] crosstie would have been opened in accordance with station

-procedure QOA 4100-2, Fire Protection System Failure. The service water crosstie cati be. electrically opened from the 912-1 panel (PL] in the control room to feed the fire protection water system. This system is capable of supplying adequate amounts of water to the fire system piping, but under normal operating conditions, at a.slightly lower pressure, 5-10 psi lower than that required by the most demanding sprinkler system (KP] [SRNK). This reduction in pressure would cause the sprinkler discharge pattern at a remote system be reduced to a small extent. To provide tha additional fire header pressure needed, the station has two backup diesel driven pumps, one of which is required by procedure to be tied into the designated fire main hydrant (HYD). Each of these pumps is rated at 140 psi which satisfies the most demanding fire system pressure requirement of 100 psi and-satisfies the Technical Specification requirement for a backup fire suppression system, l

The 1/2 B fire pump will be inspected if it is determined that the interval- since  ;

its last inspection was' the same as the 1/2 A fire pump (NTS 2542009004/C ).

F. PREVIOUS EVENTS:

There were two previous events that dealt with the-fire pumps baing declared inoperable due to component wear. Deviation Report 4-1-85-97 documented an 00S

- greater than 7 days due to worn wear rings on the B pump. DVR 4-1-85-44 also o documented worn pump wear rings on both pumps, however, the 00S did not exceed 7- i days, s Based on actions to be completed, no further action is deemed.necessary. ,

G.. COMPONENT FAILURE DATA:

~ This event was not result of component failure. However, the pump was-repaired as i needed upon inspection.
