Loss of No. 2 Station Power Transformer (Interconnect between offsite transmission network and onsite Class lE) CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 2, Reactor Power: 10-8 amps DESCRIPTION OF OCCURRENCE:

At 1140 hours0.0132 days <br />0.317 hours <br />0.00188 weeks <br />4.3377e-4 months <br />, during testing of the 1-5, 500kV GCB, the 1-5 breaker* was closed in the test position.

The 1-8, 500kV GCB opcnej as well as the 3-4 and 4-5, 13kV breakers.

This action isolated the 500kV Bus Section No. 1 and the 13kV Bus Section No. 4. The F ane G gr0up busses were de-energized and 13 and 14 Reactor Coolant Pumps fed from the G group busses were lost. Technical Specification 3.R.J.l.a was mented and surveillance 4.8.1.1.1.a and 4.8.1.1.2.a.2 was

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

The cause of this event was personnel error. During the testing of the Bus Section 1-5, 500kV breaker, the breaker and ground tion circuit was not tagged out of service. When the 1-5 breaker was closed in the test position, the protective relay ciicuit saw this as a "stuck breaker" and. is.elated the apparent fault. ANALYSIS OF OCCURRENCE:

Technical Action Statement 3.8.1.1.a allows 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to restore an inoperable offsite circuit to operable status. The No. 2 Station Power transformer was restored to operable status in 1 hour1.157407e-5 days <br />2.777778e-4 hours <br />1.653439e-6 weeks <br />3.805e-7 months <br /> and 44 minutes, therefore, the Technical Specifications were not violated.

This event did not cause any personal injury or equipment damage.

. , Report No. 77-46/03L e 7/12/77 ,. CORRECTIVE ACTION: The breaker failure and ground protection circuit was tagged out of service for the remainder of the breaker testing. The SOOkV, Bus Section 1-8 breaker and the 13kV, Bus Section 3-4 and 4-5 breakers were reclosed to restore the electrical distribution system to normal. Although the SOOkV breakers are non-safety related, a detailed cedure covering testing of switchyard components is being prepared and should be available by September 1, 1977. FAILURE DATA: Not Applicable.
