e 77-28/03L Report Date: 5/2/77 Occurrence Date: 4/18/77 Facility Salem Generating Station Public Service Electric & Gas Company Hancocks Bridge, New Jersey 08038 IDENTIFICATION OF OCCURRENCE:

Loss of New Freedom South 5024, 500kV Transmission Line.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1, 55% Reactor Power DESCRIPTION OF OCCURRENCE:

At 1550 hours0.0179 days <br />0.431 hours <br />0.00256 weeks <br />5.89775e-4 months <br />, the 2-6 and the 5-6, 500kV Bus Tie Breakers opened on protective relay actuation. This caused a loss of the 5024 500kV transmission line. This event did not cause a reduction in Reactor power or generator load because the 1-5 breaker stayed closed keeping the 5023 line energized. Technical Specification Action Statement (a) for LCO 3.8.1.1 was implemented and surveillance 4.8.1.1.1.a was com-pleted satisfactorily. The 5024 line was checked by helicopter and at 1633 hours0.0189 days <br />0.454 hours <br />0.0027 weeks <br />6.213565e-4 months <br />, re-energized. At 1634, the 2-6 breaker was reclosed and at 1635, the 5-6 breaker was closed. At this time, the 5024 was determined to be operable and the action statement terminated.

DESIGNATION OF APPARENT CAUSE OF OCCURRENCE:

A forest fire between towers 36/1 and 37/1 (36 miles from station) caused arcing from an insulator to ground. This arcing caused pro-tective relays to actuate isolating the 5024 line at the station and at New Freedom switching .station.

ANALYSIS OF OCCURRENCE:

Action Statement (a) for LCO 3.8.1.1 allows 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to restore an inoperable off-site circuit. The 5024 line was inoperable for 45 minutes, therefore, the LCO was not violated. Surveillance 4.8.1.1.1.a was completed satisfactorily but 4.8.1.1.2.a.2 was not accomplished because the 5024 line was returned to service and the Action Statement terminated. At no time was there any danger to the health and safety of the general public or site personnel.

  • 5/2/77 Investigation revealed that protective relay actuation a~ the New Freedom switching station and at the station had isolat~~ the trans-mission line. The line was checked by helicopter and nc ground fault could be found. Protective relays were reset .and the li~e returned to service.
