Loss of No. 2 Station Power Transformer (Interconnect between offsite transmission network and onsite Class lE).

  • CONDITIONS PRIOR TO OCCURRENCE:

Operational Mode 1 -Reactor Power 90% DESCRIPTION OF OCCURRENCE:

At 1453 hours0.0168 days <br />0.404 hours <br />0.0024 weeks <br />5.528665e-4 months <br />, the Gas Turbine was being placed in service when the control operator noticed that there was no electrical output from this unit. The operator stopped the gas turbine. When the gas turbine 13kV output breaker opened, *the 13kV Bus Section 4-5 also opened causing a of p9wer to No. 2 Station Power Transformer.

Vital buse: swapped over to No. 1 Station Power Transformer.

Action Statement was implemented.

At 1716 hours0.0199 days <br />0.477 hours <br />0.00284 weeks <br />6.52938e-4 months <br />, power was restored to No. 2 Station Power Transformer and applicable Action Statements were terminated.

DESIGNATION OF APPARENT CAPSE OF OCCURRENCE:

The cause of this occurrence was operator error. The 13kV Gas Turbine output breaker was found to be in the test position rather than fully racked in. Opening of this breaker in this condition caused relay operation resulting

  • in opening of the 13kV Bus Section 4-5 and loss of power to the No. 2 Station Power Transformer.

ANALYSIS OF OCCURRENCE:

Technical Specification 3.8.1.1.a allows 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br /> to restore an inoperable offsite circuit to operable status. The No. 2 Station Power Transformer was returned to operable status within 3 hours3.472222e-5 days <br />8.333333e-4 hours <br />4.960317e-6 weeks <br />1.1415e-6 months <br />, therefore, the Technical Specifications were not violated.

CORRECTIVE ACTION: Following the loss of power to No. 2 Station Power Transformer, maintenance personnel found the 13kV Gas Turbine output breaker in the test position.

Investigation showed no equipment damage and power was restored to No. 2 Station Power Transformer.

To prevent reoccurrence of this type of event, the responsible operator was reinstructed on the proper method of breaker operation.
