10 CFR PART 21 REPORT - SIEMANS-ALLIS BREAKER MIS-WIRED DURING REFURBISHMENT This letter provides written follow-up notification of a breaker which was mis-wired following refurbishment. The mis-wiring resulted in the blowing of a 30 ampere fuse which supplies DC power to the breaker. The blown fuse resulted in the loss of the breaker trip circuit and the breaker indicating lights. Consumers Power Company verbally notified BLBurgess of the USNRC, Region III, of this event on November 7, 1989.

  • The mis~wired breaker is a Siemans type MA250B 2400V breaker, serial number 24916~7. This breaker was originally removed from service and sent to Siemans Energy and Automation, PO Box 2387, 5700 W Hemlock, Milwaukee, ~isconsin 53223 in the summer of 1989 to repair a. worn plunger and threaded rod. The breaker was sent out for the replacement of the plunger which required disassembly of the breaker. During reassembly Siemans Energy reported the wrong drawing was used due fo a human error.

When the breaker was returned.to Palisades, it was reinstalled on August 10, 1989, in its original cubicle. During the post-maintenance testing, the breaker fuse blew and the breaker failed to trip. Troubleshooting by Palisades Electrical M.;iintenance personnel found the breaker.was not wired per the Palisades drawings. Following discussions with Siemans Energy, the breaker was rewired, tested and returned to service on August 15, 1989.

Subsequent engineering and procurement evaluation completed on November.7, 1989 determined the vendor used the wrong drawing.to rewire the breaker.

- This- condition was determined f() be a human -error and app~ars to be an isolated case. Palisades has previously shipped eleven (11) similar breakers to Siemans Energy on a separate purchase order and had hot identified any problems.' The vendor has corrected their drawing file for Palisades breakers and has.stated that Palisades is the only nuclear plant for which they perform service.
