10CFR21 REPORT OF ALLIS CHALMERS SWITCHGEAR BREAKERS REFURBISHED BY SITEK OF MILWAUKEE The following information is provided with respect to a 10CFR21 reportable occurrence. at Palisades. A thirty day written follow up report will be submitted.
On April 15,1992, the evaluation of improperly terminated wires on the closing relay for the 2400V switchgear breaker to P-66A, High Pressure Safety Injection Pump was determined to be 10CFR21 reportable.
On April 15,1992, the evaluation of improperly terminated wires on the closing relay for the 2400V switchgear breaker to P-66A, High Pressure Safety Injection Pump was determined to be 10CFR21 reportable. These improper wire terminations were completed during a solenoid operated breaker refurbishment by Sitek of Milwaukee. Seven 2400V solenoid operated breakers from the Palisades Plant were sent to Sitek of Milwaukee for refurbishment. We believe that all seven solenoid operated breakers have the improper wire terminations.

One of the breakers supplies power to the safety related HPSI pump, while the other six breakers supply non-safety grade equipment. To date four breakers (including the safety related one) have been inspected and fixed and all exhibited the same problem. The remaining three breakers will be inspected at a later date.
