REVIEW OF PURCHASE REQUEST FORMS INITIATED BY CONSTRUCTION -HTRD                518/81-14, -520/81 PBRD-50-553/81-15 The subject deficiencies were initially reported to NRC-0IE, Region II, Inspector P. A. Taylor on May 20, 1981 as NCR's HTAHPP81-01 and PBNHPP81-01. 'TVA submitted the first, second, and third interim reports on June 19, August 28, and November 6, 1981, respectively. In compliance with paragraph 50.55(e) of 10 CFR Part 50, we are enclosing the fourth interim report on the subject deficiencies. As discussed with P. A. Taylor on November 12, 1981, TVA was granted a 25-day extension to the reporting date indicated in our last response. TVA anticipates transmitting the next report on or before April 2, 1982. PBRD-50-553/81-15 Description of Deficiency Purchase request forms initiated by the Division of Construction (CONST) have not been processed according to Construction Engineering Procedures (CEP) 4.01. In this procedure, CONST is directed to fill out all items on the purchase request form except blanks 8,10 (items b, c, d, and part of e), and 12, which deal wi'h seismic requirements, schedule dates, and design basis verification, respectively. These items should be filled out by the appropriate lead design project engineer in the Division of Engineering Design (EN DES), as specified in EN DES Engineering Procedure (EP) 5.20. CONST has been filling out the entire form and sending it to the EN DES design project manager (DPM). The DPM's secretary, noticing that the forms were totally completed, has been sending the forms to the appropriate EN DES contract sections without having the lead design project engineer review them for accuracy and complete blanks 8, 10, and 12.
Interim Progress A batch search has been performed by MEDS to locate all purchase request forms for Phipps Bend Nuclear Plant. The results of this search are being transmitted by EN DES to CONST for their review and verification.
The disposition of QA purchase request forms for Hartsville Nuclear Plant will'b5 submitted with the Phipps Bend disposition. We anticipate transmitting our additional report on or before April 2, 1982.
