engines located at Palo Verde Nuclear Generating Station of ' i Arizona Public Services. [

4 The engine in question is known as a KSV-20-T. The twenty 1 cylinders of this engine have a bore of 13.5 inches and a -

stroke of 16.5 inches. It is pure turbocharged and rated ~-

, at 5000K at 600 RPM. The engine bears the Serial dumber -=

7187 and is known at the Palo Verde plant as Unit 111 __

Diesel "B". _

f On the morning of Tuesday, December 23, 1986, and during a orogrammed test run, the master power rod in the number 9 location broke at a fractured surface.

subsequently occurred to both the power train components and the engine structure in the imediate area.

resulted in the engine becoming totally inoperable. ,

'J A metallurgical examination of the fracture surfaces as $

well as an analysis of the chemical and physical properties y of the red forging has been completed. It is concluded -

lE that a fatigue failure originated in the rod when iron plating in the articulated rod pin bore fciled in fatigue. g<

It was also determined that the chemical and physical 9 properties of the rod forging material were within specification. The iron plating was used to correct a j dimensional error which occurred during machining. -1 d

An assessment of the damage sustained by the engine's structure and components is currently being undertaken and details of a repair program are being formulated. More information on this aspect will be sent to you when this task has been completed. It should be noted, however, that all damaged areas can be properly repaired with the engine in situ.

A thorough examination of all pertinant records revealed that three other master rods were iron plated in a manner similar to the one which has failed. One rod bearing Serial No. 8L1603 is installed in the number 2 throw of the engine which experienced the failure. A second rod, Serial No. 7E1209, is the sixth throw of engine Scrial No. 718E which is also located at Palo Verde. The third rod bears the Serial Numbe.' GE1213 and was built into the number 3 throw of engine Serial Number 7169 located at the Byron Generating Station of the Connonwealth Edison Company.

This rod has already been replaced with a new one and the engine returned to service.

This letter confirms information already transmitted to the Vendor Branch of the Nuclear Regulatory Connission.

Further details with respect to the rebuild program will be sent to you as soon as it has been completed.

Sincerely, h h -.
