.fael Nozzle Tips In accordance with the requirements of the Nuclear Regulatory Comission, and in particular 10 CFR Part 21, we wish to inform you of an incident involving one of our engines used for driving a standby power generator.
The engine in question is known as a KSV-20-T.
The twenty cylinders of this pure turbocharged diesel engine have a bore of 13.5 inches and a stroke of 16.5 inches with a rated 7860 BHP approximately at 600 RPM.
There are four such engines installed at the Byron Plant of Comonweal th Edison Co. (Ceco).
The incident being reported involved the engine bearing the serial number M0-7174 and known locally as unit 18.
A five year surveillance had been completed on the engine and it started a 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> run on September 30, 1988.
Although the run was coirpleted during the morning of October 1, local conditions required the engine to remain running beyond the 24 hour2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> test period.
During this additional running time unusually heavy vapors were observed emitting from the breathers on some cylinder head covers.
With 590 hours0.00683 days <br />0.164 hours <br />9.755291e-4 weeks <br />2.24495e-4 months <br /> total running recorded, the engine was shutdown ano Cooper-Bessemer notified as to the observed conditions.
Subsequent disassembly and test identified six fuel nozzle tips with cracks permitting diesel fuel to be sprayed into the cylinders affected in an uncontrolled manner.
This is similar to an incident which occurred 6t the South Texas Project (H. L.
and P.) last year and was covered by a 10 CFR 50.55 (e) report submitted to the Nuclear Regulatory Comission on August 26, 1987, reference ST-HL-AE-Z331, File No.: G12.399, G2.2.

OCG-5092 October 4, 1988 The fuel nozzle tips which failed at STP and CECO, Byron are from two different manufacturing lots.
We have determined that the lot number involved at Byron is 150008 and the location of the 146 pieces, part no. 2-50F-003-023, received by us is as follows:
Conmonwealth Edison, Byron 25 Commonwealth Edison, Zion 17 Houston Lighting & Power, STP 27 Arizona Nuclear Power Project, Palo Ver 31 Cooper-Bessemer In-house 46 Total 146 Concurrent with this notification, we are similarly notifying the affected Utilities.
At this moment we can only surmise that the failure mechanism parallels that of the STP experience.
As yet we have not examined any of the failed tips and therefore, have had no opportunity to determine the root cause of the failure.
- Also, therefore, we are unable to advise the final disposition for the 146 fuel nozzle tips located as shown above.
As soon as more information becomes available, we will advise you further.
