CIRCUIT BREAKER COTTER KEY MISSING This letter provides written follow-up notification of a missing cotter pin in an I.T.E. breaker trip mechanism that resulted in the breaker failing to close. The breaker is an I.T.E. K225, 480 Volt K-line circuit breaker used for charging pump breakers.

CIRCUIT BREAKER COTTER KEY MISSING This letter provides written follow-up notification of a missing cotter pin in an I.T.E. breaker trip mechanism that resulted in the breaker failing to close. The breaker is an I.T.E. K225, 480 Volt K-line circuit breaker used for charging pump breakers.
Eight of these breakers were sent to Asea Brown Boveri in Troy, Michigan for refurbishing. Following refurbishment, the breakers were returned to the Palisades Plant and installed. Thereafter, one breaker failed to the trip-free position and would not close when given a close signal. It was removed and, following comparison with other breakers, found to have the cotter pin in the tripper bar mechanism in the escutcheon assembly of the breaker trip mechanism missing. Evidence indicates that the cotter pin was never installed. The seven remaining refurbished breakers were found to have their cotter pins in place.

Eight of these breakers were sent to Asea Brown Boveri in Troy, Michigan for refurbishing. Following refurbishment, the breakers were returned to the Palisades Plant and installed. Thereafter, one breaker failed to the trip-free position and would not close when given a close signal. It was removed and, following comparison with other breakers, found to have the cotter pin in the tripper bar mechanism in the escutcheon assembly of the breaker trip mechanism missing. Evidence indicates that the cotter pin was never installed. The seven remaining refurbished breakers were found to have their cotter pins in place.

This deficiency was discovered in May of 1987, and on July 7, 1987 determined not to be reportable per 10CFR Part 21. However, following QA review, a revised reportability determination has been made. The site resident inspector was verbally notified on May 13, 1988. However, due to an over-sight, the written confirmation was not processed in the required 5-days following receipt of the notification information; i.e., in this case, following determination of reportability.

The molded assembly with the missing cotter pin was supplied to the Troy, Michigan service shop by BBC Brown Boveri, 35 North Snowdrift Road, Allentown, PA 18106.

All corrective actions were completed by July 6, 1987.
