MST-I0330 was revised to re-insert the "loss of counts test" into the procedure and maintenance procedure personnel were counseled on the incident to provide emphasis on the importance of understanding surveillance requirements in MST procedures               to prevent future recurrence.                                         In addition, Radiation Monitor HAREM-21WS-3542 will be                           tested           upon         completion     of maintenance and returned to operable status.                           A review of composite samples taken revealed no non-natural activity present in the SWST release; therefore, no significant safety consequences resulted from this event.
This event is being reported in accordance with 10CFR50.73 (a) (2) (i) (B) as a TS     violation.

On November 6, 1990, the plant was operating in Mode 1 at 100X power.Maintenance procedure personnel submitted a SOOR to the Shift Foreman in the Main Control Room.This was in regard to the requirements of TS Table 4.3-8.Upon evaluation by the Regulatory Compliance group on November 8, 1990, it was determined that section 4.3.3.10 table 4.3-8 item l.a.3 table notation (1)b.required a quarterly (once per 92 days)"loss of counts test" for Radiation Monitor REM-21WS-3542.

This corrective action required the deletion of steps in procedure MST-Z0356 that disconnected and subsequently reconnected coaxial cables.This revision was to prevent pin breakage in these cables, specifically for Radiation Monitor REM-1LT-3502A (RCS Leak Detection System).The corrective action also stated that this revision would apply to other applicable maintenance procedures.

EVEHT DESCRIPTION:
Therefore, a time period of 140 days elapsed between the last satisfactory"loss of counts test", which occurred on April 26, 1990, and the day that the monitor was declared inoperable (for maintenance) on September 13, 1990.The 92 day frequency plus 23 days (25X margin)would have elapsed on August 20, 1990.This resulted in violation of TS section 4.3.3.10 for a period of 25 days.CAUSE: The cause of this event was a misinterpretation of TS requirements on the part of the individuals revising and reviewing the MST procedure while attempting to comply with the intent of LER 90-005-00 corrective action number 5.SAPETY SIGNIFICANCE:
On     November 6, 1990, the plant was operating in Mode 1 at 100X power.
There were no significant safety consequences as a result of this event.Although the"loss of counts" surveillance test was not performed within the required frequency, the monitor was verified twice a day to be operating properly by the performance of a trend check.This check is conducted and documented by Health Physics radiation monitoring system personnel.
Maintenance procedure personnel submitted a SOOR to the Shift Foreman in the Main Control Room. This was in regard to the requirements of TS Table 4.3-8. Although the "loss of counts" surveillance test was not performed within the required frequency, the monitor was verified twice a day to be operating properly by the performance of a trend check.                                     This check is conducted and documented by Health Physics radiation monitoring system personnel.                                               Additionally, a composite sample is collected and analyzed on a weekly basis when continuous releases are made from the SWST.                           A review of these samples for the time period during which the surveillance interval was exceeded, revealed no non-natural activity present in the SWST release.

Upon evaluation of a Significant Operational Occurrence Report (SOOR) it was determined that part of the required surveillance test for the Secondary Waste Sample Tank (SWST) Radiation Monitor HAREM"21WS-3542 had not been performed within its quarterly time interval. This was due to a revisi."n of the applicable maintenance surveillance test procedure (MST-I0330 Secondary Waste Tank Dx,scharge Monitor Operational Test). This revision incorrectly deleted the "loss of counts test" portion of the procedure that was required by Technical Specification (TS)

Section 4.3.3.10 table 4.3-8 item l.a.3 table notation (1) b. As a result, the "loss of counts test" was not performed within its 92 day frequency (plus 23-days for 25K margin) and violated TS Section 4.3.3.10 for a period of 25 days. The cause of this incident was a misinterpretation of TS requirements on the part of maintenance procedure personnel. The following corrective actions were taken'.

MST-I0330 was revised to re-insert the "loss of counts test" into the procedure and maintenance procedure personnel were counseled on the incident to provide emphasis on the importance of understanding surveillance requirements in MST procedures to prevent future recurrence. In addition, Radiation Monitor HAREM-21WS-3542 will be tested upon completion of maintenance and returned to operable status. A review of composite samples taken revealed no non-natural activity present in the SWST release; therefore, no significant safety consequences resulted from this event.

This event is being reported in accordance with 10CFR50.73 (a) (2) (i) (B) as a TS violation.

EVEHT DESCRIPTION:

On November 6, 1990, the plant was operating in Mode 1 at 100X power.

Maintenance procedure personnel submitted a SOOR to the Shift Foreman in the Main Control Room. This was in regard to the requirements of TS Table 4.3-8. Upon evaluation by the Regulatory Compliance group on November 8, 1990, it was determined that section 4.3.3.10 table 4.3-8 item l.a.3 table notation (1)

b. required a quarterly (once per 92 days) "loss of counts test" for Radiation Monitor REM-21WS-3542. This test had last been performed satisfactorily on April 26, 1990, as a part of MST-I0330 (Secondary Waste Tank Discharge Monitor Operational Test). A revision to this procedure was issued on July 7, 1990, to comply with corrective action number 5 of LER-90-005. This corrective action required the deletion of steps in procedure MST-Z0356 that disconnected and subsequently reconnected coaxial cables. This revision was to prevent pin breakage in these cables, specifically for Radiation Monitor REM-1LT-3502A (RCS Leak Detection System). The corrective action also stated that this revision would apply to other applicable maintenance procedures. This corrective action was incorrectly applied to MST-I0330.

Therefore, a time period of 140 days elapsed between the last satisfactory "loss of counts test", which occurred on April 26, 1990, and the day that the monitor was declared inoperable (for maintenance) on September 13, 1990.
