IV.RESPONSIBILITIES NOTE:A.B.C.TheLeadDiscipline Engineers (ordesignee) atCHOCandtheSiteEngineering Group(SEG)responsible forinitiating achangewithinthescopeofthisprocedure (i.e.,E&OCR/ACN/N80, Speci-ficationRevision/Addenda) wi11ensurethatAttachment Aiscompleted inaccordance withSectionV,andincludedaspartoftheE&OCR/ACN/N80.

ForSpecification Revision/Addenda, Attach-mentAshallbefiledinthespecification jobbookwiththeSpecification ReviewForm.Thelead/senior engineers shallensurethatthecompleted attachment, inaccordance withSec-tionV.A,istechnically correct,consistent withlicensing requirements, andincludedaspartoftheE8DCR/ACN/N80.

ThisdoesnotapplytoE&DCRs/ACNs/N80s whichactascoverpagesforGE-NSSSFDIs,ECNs,orFODRs.Itistheresponsibility oftheleadseismicand/orenvironmental equipment qualification

engineer, asapplicable, toprovidetheEquipment gualification ActionItem(EgAI)(Attachment B)formsorconcurrence asapplicable; and,.whenrequested, provideassistance incompleting Attachment Aandprovidetechnical justifications forNOIMPACTstatements.

TheAssistant totheProjectEngineer-Equipment gualification (ATPE-Eg) willmonitorthereviewofchangeswhichaffectthequalification ofequipment andensurethatallopenandactionitemsidentified ontheEgAIarelogged,filed,andproperlydispositioned.

Itisalsotheresponsibility oftheATPE"EQtoestablish aseqentially numberedlogidentifying andtrackingtheEQAI.TheATPE-EQshallensurethattheEQAIiscompleteandproperlysignedinaccordance withthisprocedure, anddistribute theEQAIasindicated ontheform.V.PROCEDURE A.PROCEDURE FOREQUIPMENT QUALIFICATION REVIEWOFEQUIPMENT CHANGEDOCUMENTS AllE&OCRs,ACNs,N&Ds(excluding thosewithGE-FOORsattached),

and.Specification Revision/Addenda whichaddressequipment coveredbythescopeofthisprocedure (SectionII)shallbereviewed(priortoProjectEngineersignature) toevaluatetheimpactonseismic,mechanical, andenvironmental equipment qualification.

Thisreviewshallbeconducted byfollowing theinstructions onAttachment Aanddocumented byincluding thecompleted Attachment Aaspartofthechangedocument.

AllE&OCRs,ACNs,andN&Dsforequipment withinthescopeofthisprocedure musthaveAttachment AincludedaspartoftheE&DCR/ACN/N&D.ForSpecification Revision/Addenda, Attachment Ashallbeincludedwiththespecification reviewform.B.C.PROCEDURE FORREVIEWOFOTHERDOCUMENTS AllchangestotheEQEDC,PAOPsheets,PES803,anddynamicloads(RRS)shallbereviewedtoevaluatetheimpactonqualification ofequipment bytheapplicable leadEQengineerusingtheEQAIforminitiated bytheATPE-EQ.Seismic/environmental equipment qualification engineers maybecontacted whentheirassistance isneededincompleting thisattachment orjustifying theimpactofthechangeonequipment qualification.
