MULTIPLE EQUIPMENT PROBLEMS

! On August 27, 1986, the licensee declared an unusual event and shut the unit down because of equipment problems which led to the inoperability of the High Pressure Coolant Injection (HPCI) System and the "B" train of the Core Spray System. The HPCI system had been previously declared inoperable because of a gland seal leakoff pump problem; the Core Spray System's "B" train pump discharge piping was depressurized due to a partially open test

return valve. These problems have been cqrrected.

> During the shutdown additional problems were identified. .The licensee intends to take this

' opportunity to correct these and other problems that already existed. These repair efforts

..are underway. These problems included a loose wire in the output breaker for shared L Units 2/3 Diesel Generator, a failed mini-flow valve in the Low Pressure Coolant Injection i (LPCI) System, blockage b3 small clams in a LPCI Heat Exchanger, a control rod drive problem, t and a stuck source range monitor.

Licensee Corporate management is on site to review the plant's status prior to a restart decision.
