01)ERATIONAL SHARY 00I0BER1990 UNIT 1 Unit _1 generated 461,390 MWh (gross) electrical power during October, with a capacity factor of 52.35 percent. Unit 1 was operating at 100 percent reactor power level at the beginning of October, but was forced to shutdown on October 8, 1990. At 0710 (EDT) that day, the unit entered Limiting Condition for Operation (LCO) 3.7.1.5 and a notice of unusual event (NOUE) was declared as a result of identifying a disc failure by radiograph on Main Steam Check Valve (MSCV) 1-VLV-1-624. Following shutdown of the unit, it was found that three MSCV disc post failures had occurred, 1-VLV-1-623, 1-VLV-1-624, and 1-VLV-1-625. The turbine was manually tripped at 1052 (EDT); and at 1102 (EDT), the unit entered Mode 3. Unit 1 entered Mode 4 on October 8, 1990, at 1535 (EDT), and remained in Mode 4 throughout the process of repair / replacement on the check valves. Full details of the forced Unit 1 shutdown are documented in Incident Investigation Report II-90-117 and LER 50-327/90024.

On October 21, 1990, at 0800 (EDT), Unit 1 was again critical, and power level increase began. The unit was online again at 2254 (EDT). Unit i reached 99 percent power using delta T indicators on October 23, 1990, at 1130 (EDT),

and was at 100 percent on October 24,1990, at 1605 -(EDT).

Unit 1 was operating at 100 percent reactor power level at the end of October.

UNIT 2

-Unit 2 Cycle 4 refueling outage continued throughout the month of October.

Unit 2 began the month in "no mode," i.e. , fuel of floaded f rom the reactor vessel. On October 8, 1990, the unit entered Mode 6 when the first fuel assembly was reloaded into the reactor vessel. On October 11, 1990, at 0445 (EDT), all fuel was reloaded. On October 15, 1990, at 1025 (EDT), Unit 2 entered Mode 5.and remained in Mode 5 through the end of October.

= POWER-OPERATED RELIEF VALVES (PORVs) AND SAFETY VALVES

SUMMARY

There were no challenges to PORVs or safety valves in October.

The failure mechanism was the high-cycle, low-stress fatigue of the post Caused by reverse bending. The turbine was manually tripped at 1952 EDI, and at 1102 EDT. Unit 1 entered Mode 3. Unit I entered Mode 4 on October 8, 1990, at 1535 EDT, and remained in Mode 4 throughout the process of repair / replacement on the check valves. To prevent recurrence, modifications were made to

_ improve stress distribution ar.d reduce static tensile residual stresses on the MSCV disc post. On October 21. 1990, at 0600 EDT, Unit I was again critical and was online at 2254 EDT. Unit I was at 100 percent reactor power level on October 24, 1990 at 1605 EDT.
