REPORT NOS. 50-327/86-45 AND 50-328/86-45

On July 21-25, 1986, the NRC staff conducted a special inspection to examine

TVA's Design Baseline and Verification Program (DBVP) for the Sequoyah Nuclear

Power Plant at the Division of Nuclear Engineering offices in Knoxville,.

Tennessee.

The NRC inspection comprised a multi-discipline team of personnel

from the Office of Inspection and Enforcement, the Office of Nuclear Reactor

Regulation, consultants, and the Region II Office.

The team concentrated on the various design basis documents to be used in the

DBVP, including the commitment / requirement data base and both system and

general design criteria.

The team also reviewed program procedures and plans

and the action items resulting from independent oversight of this program by

TVA's Engineering Assurance group.

As a result of the inspection, the NRC team concluded that the preparations and

approach for the program were in most cases in accordance with the program

plan, subject to the following general-comments.

The Engineering Assurance oversight was not as thorough or as technically

oriented as anticipated by the inspection team.

Increased Engineering Assur-

ance oversight is needed to verify the accuracy of the commitment / require-

ment data base and inclusion of commitments and requirements in the design

criteria.

The team identified several cases where commitments and requirements applicable

to selected design criteria were not captured.

Environmental Qualification

requirements were found to be incorporated in an inconsistent manner among

various design criteria.

The team also identified missing requirements in the

design criteria for the neutron monitoring system, apparently at the interface

between the NSSS and TVA.

These findings, collectively, raise a concern

regarding the comprehensiveness of the design criteria.

The computerized list of licensing commitments and design requirements called

the " Commitment / Requirement Data Base," developed by TVA/Impell, was not

independently verified.

to the information retrieval process, the Engineering Assurance group had

conducted reviews in this area on a sampling basis which we consider to be

inadequate. This data base forms a large portion of the basis for the newly

issued design criteria documents which in turn will be used by TVA to evaluate

~

the technical adequacy of design changes in the scope of the DBVP. Therefore,-

TVA should clearly define their basis for concluding that the commitment /

requirement data base have accurately captured all licensing commitments,

regulatory requirements and other pertinent design information as applicable,

e.g., NSSS/ vendor interface requirements, and that the appropriate commitments

and requirements have been incorporated in the applicable design criteria.

Although no written response is requested from you addressing the observations

identified in the enclosed report, the NRC intends to follow your. actions to

resolve these items during future inspections. The findings addressed in the

. enclosure were discussed by the team in a meeting with TVA management at the

conclusion of the inspection.

In accordance with 10 CFR 2.790(a), a copy of

this letter and the enclosure will be placed in the NRC Public Document Room.
