F APPENDIX NOTICE OF VIOLATION Houston Lighting & Power Company Docket: 50-498 South Texas Project (STP), Unit 1 Operating License: NPF-76  ;

Ouring an NRC inspection conducted on July 11-15 and 25-29 and August 8-12, 1988, a violation of NRC requirements was identified. The violation involved failure to have instructions or procedures to include appropriate quantitative or qualitative acceptance criteria for determining that ir ortant activities have been satisfactorily accomplished. In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1988), the violation is listed below:

A. Failure to Have Instructions or Procedures to include Appropriate  :

Quantitative Acceptance Criteria for Detennining that Important Activities l Have Been Satisfactorily Accomplished 10 CFR Part 50, Appendix B.Section V. "Instructions Procedures, and Drewings," requires that "Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate i to the circumstances and shall be accomplist ed in accordance with these ,

instructions, procedures, or drawings. Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been ,

satisfactorily accomplished." l This requirement ir amplified by the licensee's Quality Assurance Plan ,

requirements of FSAR Section 17.2.5 (Instructions, Procedures, and Drawings), Subsection 17.25.2 (Acceptance Criteria). ,

Station Procedure, OPG P03-ZM-003 "Maintenance Work Request Program,"

paragraph 4.13.9, requires that the Maintenance Support Division "Enter ,

document numbers left open in Section X of WR." Paragraph 4.6.1, howev:r, #

does not recognize an WR as a valid document until it has been approved by the (work start) approval authority.

Contrary to the above, adequate procedures were not in place to assure that ,'

important activities had been satisfactorily accomplished. WR HE-69441 was closed between June 8 and June 24, 1988, without replacing the ElC11 l Bittery Room Duct Heater Panel Bolts. The "as left" conditions of Section X ,

of WR PE-69441 listed a new WR, No. 45065, as initiated. The licensee's '

records indicate that WR 45065 was initiated between June 8-10, 1988, but i subsequently cancelled on or before June 14, 1988, prior to becoming a (

valid document with approved (work start) authority. The battery room l duct heater panel bolts remained in storage until this fact was brought to l the attention of the licensee by the NRC inspector on July 27, 1988. j l

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit to this office, within 30 days of the date of the letter transmitting this Notice, a written statement or explanation in reply, including for each violation: (1) the reason for the violation if aOnitted,

2) the corrective steps which have been taken and the results achieved, 3)thecorrectivestepswhichwillbetakentoavoidfurtherviolations,and
4) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending the response time.
