,10 CFR Part 50, Appendix B, Criterion:Y, requires that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings. ,

a South Texas Project Station Procedure OPGP03-ZA-0039, Revision 8, " Plant Procedures Writer's Guide," Section 3.2.20, " Independent Verification," -t requires, in part, that alignment changes or_ alterations in_ status of components / systems identified as safety-related be independently verified-when a' component / system is returned to service following_a period when the component / system was inoperable due to maintenance or testing or when a component / system is removed from service or altered from its normal configuration, n ,

Contrary to the' above, Station Procedure OTEP03-FP-0006, Revision 0, ,

" Relay Room Halon Concentration Test," did not include provisions for independent' verification that the control room envelope makeup fan air inlets had beenr.estored to normal configuration after testing. This oversight ' led to the plastic sheeting being left on the Train C control room envelope makeup fan inlet thereby rendering that train inoperable.

3) the corrective steps which will be taken to avoid further violations, and 4).the date when full compliance will be achieved.
