During an NRC inspection conducted August 2-6, 1993, a violation of NRC requirements was identified.
The violation involved failure to conduct training and requalification of emergency response organization personnel in accordance with your Emergency Plan.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (1991), the violation is listed below:
i 10 CFR 50.54(q) requires, in part, that licensees authorized to operate a nuclear power reactor shall follow and maintain in effect emergency plans which meet the standards in 50.47(b) and the requirements in Appendix E.

t South Texas Project Emergency Plan, Revision 15, Section M.2.3 states, I
in part, that annually, station emergency response personnel shall requalify for their position. The requalification' may be accomplished by either classroom instiuction or through successful accomplishment of position duties through the drill and exercise program.
Contrary to the above, since March 19, 1992, a large fraction of the
[
personnel ' assigned to the emergency response organization has been requalified for their positions by other means not specified in the emergency plan. Specifically, some of these personnel have been requalified by challenging a 20 question multiple choice emergency preparedness examination, achieving a passing score of at least 70 percent. A larger number of personnel were requalified by acting as an evaluator or controller in an emergency drill or exercise without having to successfully demonstrate their position duties.
This is a Severity Level IV violatim (Supplement VIII)

Pursuant to the provisions of 10 CFR 2.201, Houston Lighting & Power Company is hereby required to submit a written statement or explanation to the U.S.
Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C.
20555 with a copy to the Regional Administrator, Region IV, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of i
Violation" and should include:
(1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full t

If an adequate reply is not received within the i
time specified by this Notice, an order may be issued to show cause why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, i
