Report of 10CFR50.55(e) Item on Poor Quality of vendor Coraection Workmanship in Emergency Diesel Generator Con-trol Panels (#112)

Dear Mr. Keppler:

On January 13, 1984, Detroit Edison's Mr. W.R. Wingfield, Quality Engineer - Construction Quality Assurance, telephoned Mr. J.E.

Konklin of NRC Region III to report on poor quality vendor crimper.1 wiring connections in an Emergency Diesel Generator Control Panel.

Startup personnel performing a Field Modification Request (FMR) dis-covered loose crimp type lugs installed in the exciter regulater control circuit in Control Panel R3000S008 for Emergency Diesel Generator #14. Two connections were identified in a Nonconformance Report (NCR) by Startup. One connection on a #2 AWG wire to a shunt resistor, and one connection on a #6 ANG jumper between two silicon f control rectifiers were initially identified. The extent of~similar conditions in the other three Emergency Diesel Generator Control Panels was not known or addressed in this NCR. However, Project Quality Assurance (POA) was Ware of one previous NCR on two loose f

connections on vendor wiring (size #10 ANG or smaller) to selector f switches on Control Panel R3000S005 for Energency Diesel Generator

! #11 which.had been considered an isolated occurrence. Based on this second discovery of vendor connection problems, PDA evaluated the occurrence as evidence of a potentiul for problems associated with vendor connections.

Subsequently, on February 1, 1984, Detroit Edison's Mr. R. McMechan,

Hawkins of NRC Region III to report additicnal information related to the intentially reportable item on poor quality vendor wiring connections report on January 13. 1984, as noted above.

While observing the implementation of the disposition of the noncon- q forming condition, the NRC Site Resident Inspector discovered a g I

Keppler, Regional Administrator February 17, 1984 QA-84-0082 Page 2 loose, unlugged, solder-post type vendor connection at resistor R1 of the field flashing resistLr bank in the Control Panel R3000S005 for Bnergency Diesel Generator #11. The condition was noted by Startup and PQA inspectors present. Startup personnel initiated an

.NCR. Investigation by PQA personnel revealed one or two soldered connections of questionable workmanship per panel.

Detroit Ediron 'is currently inspecting all crimped lug connections

. of wiring instatled in the four Bnergency Diesel Generator Associ-ated Control Panels and skid nounted terminal boxes, instruments, equignent, and engine gauge panels. Inspection of each connection, and inspection of resultant connection rework are being documented.

In addition, inspection of soldered wiring connections (unlugged) and associated rework is being performed and documented for the four Energency Diesel Generator control panels. These actions address all connections originally installed by Beloit Power Systans.
