Report Detailing Failure of Telemecanique NEMA Size S Contactor, Cat. #A103G12 l
Pursuant to 10 CFR 21 Para. 21.21 (b).(3) l
Dear Mr. Finkel:
On January 16, 1986 the Rocketdyne Division of Rockwell International notified Telemecanique Inc. that during assembly performed by Rocket-dyne, two carrier assemblies from NEliA Size 5 Contactor #A103G12 had been found broken preventing proper closing of the contactor.
This' breakage was following a similar pattern as the one previously reported l
by Rocketdyne in July 1985. At that time, Telemecanique could not find any particular reason for the breakage of the device.
Following the January report, mechanical and electrical testing by Telemecanique Inc. led to the conclusion that the breakage could j
only be duplicated by dropping the device in its shipping carton.
I Af ter modification of the packaging (addition of a cushioning material) l the breakage could not be duplicated.
As a result, packaging for all NEMA Size 5 contactors and starters has been modified, as of February 5,1986.
All equipment installed, or still in their shipping cartons at Rocketdyne, has been inspected.
Telemecanique Inc. has researched their shipment files for possible I,
shipments of loose starters or contactors to nuclear sites.
One I
shipment of three contactors Cat. #A103G12 to Detroit Edison, Fermi II, has been identified and this company has beer notified.
