" Poor Quality of Vondor Connection Workmanship in Emergency Diesel Generator control Panels" This is Detroit Edison's final report of Item 112, " Poor Quality of Vendor Connection Workmanship in Emorgency Diosol Generator Control Panels". Item 112 was originally reported as a potential deficiency on January 13, 1984, and was subsequently documented in Reference (2).

Description of Deficiency Startup personnel performing a Field Modification Request (FMR) discovered two loose crimp type lugs installed in the excitor regulator control circuit in control panol R3000S000 for Emergency Diosol Generator (EDG) #14. During the ovaluation of this deficiency, Project Quality Anourance

( PQA ) identified an earlier NCR which documented two loono connections on vendor wiring to selector switchen on control panol R3000S005 for EDG #11.

This earlier NCR had boon connidored an isolated occurronco Based on this accond discovery of vondor connection problems, PQA evaluated the occurrence an ovidence of a potontial for generic problems ansociated with Er.G vendor wiro connections. As a renuit of additional investigativo actions, PQA observed that about 700 defectivo terminations had boon previously identified, reworked, and inspected under a program that had not boon reviewod by Quality NOV131984 0411260471 041031 PDR ADOCK 05000341 1 8 pan g g A 'r, X )_.

Mr. James G. Koppler October 31, 1984 EF2-70030 Pago 2 Assurance. As a result of this, additional inspections of crimped lug connections on wiring installed in the four emergency diosol generators was conducted.

Subsequently, a loose, unlugged, solder-post type vendor connection was observed at resistor R1 of the floid flashing rosintor bank in control panoi R30008005 for EDG #11. This deficiency was also documented on an NCR.

Analysis of Safety Implicationn Improper wiring connections in the omorgency diosol genor-ator electrical control circuito could provent the affected omorgoney diosol generator from fulfilling its intended safoty function.

Correctivo Action Problems with vendor installed connections on the omorgoney diosol generators woro corrected as followns o The deficient wiring terminations documented on the NCRs woro roterminated and inspected.

o Detroit Edison inspected the crimped lug connections on wiring installed in the four omorgoney dional generator control panoin and skid mounted terminal boxon, instru-monta, equipment, and engine gaugo pannin. Of the approximately 8,000 connections Inspectod, 828 woro not natisfactory and were reworked. (Thin in in addition to the 700 roworkod terminations discunned abovo).

While a limitod number of lugn exhibited slippago in the lug barrol when pulled, tho majority of the lugs woro reworked for broken or minning wiro stranda, i.e.,

not all strandn of a conductor wiro woro crimped in the lug.

o Soldorod wiring connoctions (unlugged) in the four omorgoney dional gonorator control pannin woro inspected. Of the moro than 200 connectionn innpoctod, 5 woro not natinfactory and woro reworkod.
