The Fermi 2 Nuclear Power Plant site is located on the western shore of Lake Erie in Frenchtown Township, Monroe County, Michigan. The Nuclear Steam Supply System is a General Electric BWR 4 with a Mark I pressure-suppression containment. The plant is fully owned by The Detroit Edison Company.

2.0 _S_gm_ mary of Ooerations 2.1 Summary of Ooeratine Experience Fermi 2 was shutdown at the start of 1997 due to a forced outage that occurred at -

. the end of 1996. A subsequent forced shutdown occurred on January 17,1997, due

~

to failure of the main generator output breaker while attempting to synchronize to the grid during startup from the forced outage that began on January 10,1997.

Plant restart occurred on May 2,1997. Power was reduced on May 12,199' to permit flux suppression testing for determining the exact location of the fuel leak detected in~ the reactor. Power was reduced again on September 24,1097 to permit i power suppression testing to help determine the locat n of a second leaking fuel l rod. The reactor was shutdown from October 3 througn "ober 19,1997 for a planned mid-cycle outage to replace two leaking fuel bundles and a Reactor )

Recirculation pump seal. After the mid-cycle outage, the reactor maintained power j operation with three power reductions to set Reactor Recirculation Motor Generator 1 (MG) Set limiters and stops,345 kV switchyard corrective maintenance, and main turbine stop, control, and bypass valve surveillances, control rod scram time testing, - :

control rod pattern adjustments, and reactor water level control system repair. Core  ;

Thermal Power (CTP) is administratively limited to 96 percent due to the high , j pressure turbine flow passing limitations.

2.2 Summary of Outanes and Forced Reductions Greater than 20 Percent of Full Power January 6,1997- Power Reduction Unit load reduced and main turbine generator tripped to add balance weights l to the main turbine.

January 10 - 16,1997 - 157.6 Hours Shutdown

- Reactor shutdown to repair high water level in 20 inch offgas manifold. Main Turbine remained shutdown from January 6,1997.

September 5,1997 - Power Reduction Reactor power was reduced to 76% CTP to repair Main Generator 'CM' breaker 'Z' connection hot spot.

September 24,1997 - Power Reduction Reactor power was reduced to 55% CTP to perform power suppression testing for leaking fuel rod.

J Enclosure NRC-98-0021 Page 5 October 3 - 19,1997 - 368.7 Hour Shutdown Reactor shutdown to replace 2 leaking fuel bundles and replace a Reactor Recirculation pump seal.

October 29,1997 - Power Reduction Reduced reactor power to 60% CTP to set Reactor Recirculation MG Set limiters and stops, and also to perform corrective maintenance on 345 KV Brownstown line breaker hot spots.

December 13,1997 - Power Reduction Reduced reactor power to 65% CTP to perform main turbine stop, control and .

bypass valve surveillances, control rod scram time testing and control rod pattern adjustments.

December 29,1997 - Power Reduction Reduced reactor power to 25% CTP to repair reactor water level control system.

2.3 Fuel Performance The reactor was shutdown at the start of 1997, but produced 206 effective full  ;

power days during the year. Except for a brief period during November when core thermal power was raised to 98.6 percent for power uprate testing, power was administratively limited to 96 percent due to high pressure turbine flow passing l limitations.  !

During the cycle's first approach to near rated power, which occurred during May, l off-gas activity spiked at 1,300 pCi/sec. Subsequently, power suppression testing i was performed and confirmed the presence of a leaking fuel rod. Three control rods I were inserted to reduce power in the fuel bundle and off-gas activity dropped to 400 Ci/sec. Activity levels continued to rise slowly during the next few months reaching a level of about 3,000 pCi/sec by mid-September. At that time, off-gas l activity spiked at just over 4,000 Ci/sec indicating a second leaking fuel rod. The  ;

second leaking fuel rod was confirmed by power suppression testing, and a single  !

control rod was iuserted to suppress the power in this fuel bundle.

Early in October the plant was shutdown to replace the leaking fuel. During this  ;

outage, General Electric performed both in-core sipping and vacuum sipping to confirm the number and location of the leaking fuel assemblies. The first leaking bundle was YJ7119. This bundle was in its second cycle of duty. The second leaking bundle was YJ2802 which was in its third cycle of duty.

Since the off-gas activity had been slowly rising despite power suppression for the first leaking rod, the degree of degradation was expected to be greater than a pin hole leak. This was confirmed in November during inspection of the bundle. The

, cause of the leak could not be determined. The cause of the second leak, which was

- a pm hole, was determined to be debris.

l The plant operated fuel failure free during the remainder of the year.

2.4 Shore Barrier Survey A survey of the Fermi 2 shore barrier was completed as required by Technical Specification 4.7.3. The results of the survey indicated no damage, significant movement, or deterioration of the barrier. All forty-seven survey point elevations were within the tolerance specified in Technical Specification Table 3.7.3-1. Civil Engineering Drawings 6C721-44 through 49 were revised to incorporate the survey data. No unusual incidents occurred in 1997 that would have required additional surveillances.

2.5 Safety Relief Valve Challenges There were no instances in 1997 where reactor pressure was high enough to require Safety Relief Valve (SRV) actuation. There were no instances in 1997 where an SRV actuation was demanded by an automatic logic system. SRVs were manually actuated for surveillance / post-maintenance testing as described below:

2.7 ' Service Life of Main Steam Bvoass Line -

In accordance_with a Detroit Edison letter to the NRC dated November 7,1986  !

[VP-86-0154], the cumulative time the main steam bypass lines are operated with the bypass valves between 30 percent and 45 percent open will be reported annually. A cumulative value of 100 days is not to be exceeded without prior NRC notification.

l Evaluations performed by Stone and Webster and by Hopper and Associates concluded that the bypass lines are acceptable for safe operation when operated within the 100 day constraint. Based on these evaluations, the new main steam bypass piping that was installed in 1985 has a service life which will allow it to function for the life of the plant under anticipated operating conditions. The main steam bypass lines cumulative usage was 36.70 days as of December 31,1997.
