Intrdduction The Fermi 2 Nuclear Power Plant site is located on the western shore of Lake Erie in i
Frenchtown Township, Monroe County, Michigan. The Nuclear Steam Supply System is

a General Electric BWR 4 with a Mark I pressure-suppression containment. The plant is fully owned by The Detroit Edison Company.

2.0 -
Summary of ODerations l
i 2.1 Summary of ODeratine Experience Fermi 2 was operating at 96% reactor power at the start of 1998. Fermi 2 had generated a record amount of electricity in January 1998. The 827,891 megawatt -
hours produced was the most gene;ated for any month in the history of the plant.
On February 1,1998, a reactor scram occurred that was initiated from a Main Turbine Trip caused by relay testing on the 345 kV transmission mat. The l
shutdown was caused by a faulty relay and voltage irregularities, which caused the generator output breakers to open and initiate a turbine trip signal. Investigation into the causes and corrective actions were completed and plant restan occurred on Febmey 15,1998. As scheduled, power was reduced on April 18,1998, to perform main turbine stop, control and bypass valve surveillances, control rod scram time surveillances, and control rod pattern adjustments. Power was reduced on May 4,1998, to perform feedwater and reactor water level control i'
troubleshooting and monitoring. Power was reduced again, as scheduled, on May 29,1998, to perform control rod scram solenoid pilot valve replacement for 39 control rods. The last scheduled power reduction greater than 20% full power occurred on July 19,1998. A reactor power reduction was conunenced in preparation for making a control rod pattern adjustment. With the reactor at
..approximately 64% power, reactor power began fluctuating between 50 and 75%.
A manual scram was inserted following the onset of power fluctuations. Plant

restart occurred on July 24,1998 with the No. 4 High Pressure Turbine Control
{
Valve closed for the remainder of the cycle. The reactor was shutdown from

' September 4 through October 29,1998 for the sixth refueling outage. The plant
{
restarted on October 29,1998. Power was reduced on November 5,1998 when j
reactor recirculation 'A' pump speed decreased to minimum speed while attempting I
to remove the controller from saturation. The reactor was shutdown from November 13 through 18,1998, to repair an indicator shaft pressure seal leak on a feedwater check valve. The plant restaned on November 18,1998 and maintained power operation for the remainder of the year with no shutdowns or power reductions greater than 20% full power.

l 2.3 Fuel Performance The reactor has operated failure free following the replacement of two leaking bundles during the cy:le six mid-cycle outage in October 1997. As of February 1999, off gas release rates are approximately 2500 Ci/sec, compared to approximately 7000 pCi/sec prior to the sixth refueling outage. While all leaking 4
bundles have been removed from the core, elevated off gas radiation levels and release rates remain due to residual plate-out of fuel inside the core. Off gas levels u
will continue to decrease as this plate-out is burned up and bundles are replaced in j
subsequent reloads.

' Past irradiated fuel inspections of both failed and sound bundles have revealed the presence of small metallic debris. Three of Fermi's six fuel failures are confirmed debris induced fretting failures.. For this reason a number of debris mitigation efforts have been pursued. The cycle seven reload bundles contain a debris resistant
- lower tie plate.. A significant effort continues.to increase plant Foreign Material j

Exclusion (FME) awareness and reinforce exclusion practices. The plant has also j
incorporated a " soft" operating philosophy that limits the rate ofincrease in bundle t
. power and enforces a maximum nodal power level. This " soft" operation is j
believed to reduce the risk of failure for bundles that have been damaged by debris j
fretting.
l 2.4 _ Shore Barrier Survey A survey of the Fermi 2 shore barrier was completed as required by Technical I
Specification 4.7.3. The results of the survey indicated no damage, significant movement, or deterioration of the barrier. All forty-seven survey point elevations were within the tolerance specified in Technical Specification Table 3.7.3-1. Civil Engineering Drawings 6C721-44 through 49 were revised to incorporate the survey j
data. No unusualincidents occurred in 1998 that would have required additional surveillances.
j 2'5 ' Safety Relief Valve Challences There were no instances in 1998 where reactor pressure was high enough to require Safety Relief Valve (SRV) actuation. There were no instances in 1998 where an SRV actuation was demanded by an automatic logic system. SRVs were manually actuated for surveillance / post-maintenance testing as described below:

NRC-99-0013 Page 7 2.7f Service Life of Main Steam Bvoass Line In accordance with a Detroit Edison letter to the NRC dated November 7,1986
[VP-86-0154], the cumulative time the main steam bypass lines are operated with the bypass valves between 30 percent and 45 percent open will be reported annually.
A cuinulative value of 100 days is not to be exceeded without prior NRC _
notification.
Evalitations performed by Stone and Webster and by Hopper and Associates -
concluded that the bypass lines are acceptable for safe operation when operated
. within the 100 day constraint. Based on these evaluations, the new main steam i
bypass piping that was installed in 1985 has a service life which will allow it to i
function for the life of the plant under anticipated operating conditions. The mam steam bypass lines cumulative usage was 37.38 days as of December 31,1998.

i Core Cooling System (ECCS) outages which occurred between January 1,1998 and December 31,1998 is provided. The tabulation of ECCS outage hours (Table 2.10-
- 1) includes both forced and planned outages for the Low Pressure Coolant Injection
_ (LPCI), Core Spray, High Pressure Coolant Injection (HPCI), and Automatic Depressurization Systems (ADS). An outage was considered to be whenever one of the ECCS systems was out-of-service at a time it was required to be operable per i
the Technical Specifications.
