Quality Engineer - Construction Quality Assurance telephoned

! Mr. R.C. Knop of NRC Region III to leport a proble with the RHR 4160V switchgear installation records at the Fermi 2 site.

f Detroit Edison has been unable to locate the installation records for the RHR 4160V mounting channels and anchor bolts. After exten-sive review, it appears that the mounting installation records do not exist.

On December 16, 1983, a Nonconformance Report (NCR #83-1252) was initiated to identify the deviation.

Detroit Edison Engineering inspected the existing cnchors and determined that it is necessary to provide additional anchoring for all four units. This consists of ignoring the two middle channele, chipping away the finished concrete surface from the two outer channels, adding sections of heavy duty angle iron supported by shim stock and anchor bolting to the floor. The angle iron is then welded to the outside edge of the two cuter channels. During the repair work it was identified that the existing wedge anchors used to support the mounting channels had nute on top and on bottcm.

Due to this configuration, it is probable that the supporting wedge anchors were also used for leveling the channels prior to pouring the finished concrete surface. The use of wedge anchors for level-ing is not in accordance with site requirements. This nonconfer-mance was reported to Mr. J. Konklin, NRC Region III as additional information on 10CFR50.55(e) item #108 on January 10, 1984. It was also added to NCR #83-1252 for Engineering disposition.

Detroit Edison Engineering is in the process of supplying Project Quality Assurance with a list of electrical equipnent, and the latest design drawings that depict the installation configuration.

Project Quality will then review the drawings against the installa-tion records available in order to detemine the scope of the prob-lem.
